In-Clinic Services (16 hours/week): Provide medical care to Level 2 patients in a rehab unit at the HSE Nursing Home in Gorey, Wexford.</p>\n<p>2. Remote Teleclinic Services: Deliver high-quality patient care through virtual consultations.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Key Responsibilities</span></p>\n<p>In-Clinic: Level 2 Rehab Unit Services – Ballinasloe, Galway</p>\n<p><span style=\"font-weight: bold\">Patient Care and Management</span></p>\n<p>· Conduct regular ward rounds to assess progress and update care plans.</p>\n<p>· Perform comprehensive medical assessments for new admissions.</p>\n<p>· Review and manage treatment plans, ensuring alignment with rehabilitation goals.</p>\n<p>· Oversee medication management, including prescribing, adjusting, and monitoring.</p>\n<p>· Address acute medical issues and provide immediate care when required.</p>\n<p><span style=\"font-weight: bold\">Rehabilitation Focus</span></p>\n<p>· Collaborate with the multidisciplinary team (MDT) to create individualised rehabilitation plans.</p>\n<p>· Support patients in achieving mobility, independence, and recovery goals.</p>\n<p>· Monitor progress toward rehabilitation milestones and adjust plans as needed.</p>\n<p>Coordination with Nursing and Allied Health Staff</p>\n<p>· Participate in MDT meetings to discuss patient care and progress.</p>\n<p>· Provide guidance on medical aspects of care to nurses and allied health professionals.</p>\n<p>· Ensure clear communication of care plans, medications, and any changes in patient status.</p>\n<p><span style=\"font-weight: bold\">Documentation and Reporting</span></p>\n<p>· Maintain accurate patient records using Digicare and Epicare software.</p>\n<p>· Document clinical decisions, treatment plans, and rehabilitation outcomes.</p>\n<p>· Generate reports on patient outcomes and progress for stakeholders.</p>\n<p>· Ensure all data complies with data protection policies.</p>\n<p>Family and Patient Engagement</p>\n<p>· Communicate care plans and address concerns with patients and families.</p>\n<p>· Provide education on health conditions, rehabilitation goals, and lifestyle modifications.</p>\n<p><span style=\"font-weight: bold\">Compliance and Quality Assurance</span></p>\n<p>· Adhere to clinical guidelines, protocols, and ethical standards aligned with Cork University Hospital (CUH) guidance.</p>\n<p>· Participate in audits and quality assurance initiatives.</p>\n<p>· Stay informed on best practices in rehabilitation and geriatric care.</p>\n<p>Support Services Coordination</p>\n<p>· Liaise with external specialists and ensure continuity of care.</p>\n<p>· Facilitate referrals for specialised treatments or diagnostics.</p>\n<p><span style=\"font-weight: bold\">Emergency Response</span></p>\n<p>· Provide immediate medical attention during emergencies.</p>\n<p>· Coordinate transfers to acute care facilities as needed.</p>\n<p><span style=\"font-weight: bold\">Education and Training</span></p>\n<p>· Support nursing and allied health staff on rehabilitation care.</p>\n<p>· Participate in training sessions for software systems and patient care enhancements.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Why Join Spectrum Health?</span></p>\n<p>This is a unique opportunity to contribute to the care of rehabilitation patients in a well-supported environment.
