Duties may include hospice patient visits, admissions, responding to urgent night and weekend patient needs, and/or filling in for other nursing staff. The On-Call RN works from 5:00 PM to 8:00 AM Monday-Thursday, and 24 hours on weekends Friday-Sunday, following a rotating schedule of 7 days on and 7 days off.</p>\r\n\r\n<p><span style=\"font-size:14px;\"><strong>Essential Functions and Skills</strong></span><br />\r\n<strong>Patient Care</strong></p>\r\n\r\n<ul>\r\n\t<li>Available to receive and respond to calls from Triage during assigned shifts making visits to patients with changing conditions and completing weekend admissions.</li>\r\n\t<li>Completes comprehensive and ongoing assessment of patient and family to determine hospice needs for after-hours calls. Provides a complete physical assessment and history of current and previous illnesses.</li>\r\n\t<li>Provides professional nursing care by utilizing all elements of the nursing process.</li>\r\n\t<li>Assesses and evaluates patient&rsquo;s status by:</li>\r\n\t<li>Writing and initiating Plan of Care</li>\r\n\t<li>Re-evaluating patient and family/caregiver needs</li>\r\n\t<li>Participating in revising the plan of care as necessary</li>\r\n\t<li>Initiates the plan of care and makes necessary revisions as patient status and needs change.</li>\r\n\t<li>Uses health assessment data to determine nursing diagnosis.</li>\r\n\t<li>Develops a plan of care that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.</li>\r\n\t<li>Initiates appropriate preventive and rehabilitative nursing procedures.
