If you’re searching for something more than just a job, something akin to a calling, then consider the challenge and opportunity of being a member of Johnson County Government!</p>\n<p>Johnson County Mental Health is looking to fill the position of Case Manager to work with clients who have mental health needs and are associated with programs at the Adult Residential Center.  The position provides Case Management to individuals with a mental illness, utilizing Strengths Based Case Management.  This position will work primarily from the Adult Residential Center at New Century, servicing individuals in Problem-Solving Beds and Work Release Program and will include providing case management to individuals with co-occurring mental health and substance use disorders in the community. Position will remain open until filled.  <strong>First review of applications will be May 18th. </strong></p>\n<h2>Job Description</h2>\n<p>Job duties includes:</p>\n<ul>\n <li>Conducts assessments of individuals' background and applicable circumstances to identify needs and/or determine service eligibility; interprets and communicates relevant information to individuals, families, and other stakeholders.</li>\n <li>Develops plans to address the needs of individuals in single or multiple areas of focus; networks with other community agencies to research and coordinate a variety of resources and services to address short term intensive or long-term individual needs; encourages the establishment and maintenance of support systems; consults and collaborates with other providers in the development, implementation and review of plans.</li>\n <li>Provides a variety of services for a designated case load of individuals with multi-faceted and/or ongoing needs; ensures services provided are consistent with individual plans; initiates and attends individual and/or group meetings and provides services in a community-based setting; provides transportation of individuals according to the needs identified in plans; organizes and facilitates skill development and support groups.</li>\n <li>Completes and submits documentation regarding services provided; evaluates and documents individuals' progress according to measurable goals; reviews quality and effectiveness of services provided.</li>\n</ul>\n<h2>Job Requirements</h2>\n<p><strong><u>Minimum Job Requirements:</u></strong></p>\n<p>Bachelor’s degree in Human Services or related field is required.  May require KS/MO driver’s license, depending on department or agency.</p>\n<p> </p>\n<p><strong><em>Johnson County Government requires reference/background screening for all positions.  Specified criteria may vary by Department/Agency. </em></strong></p>\n<p><strong> </strong></p>\n<p><strong><u>Preferred Job Requirements:</u></strong></p>\n<p>One year of related work experience is preferred.</p>\n<p><strong> </strong></p>\n<p><strong><u>Education/Experience Substitutions:</u></strong></p>\n<p>Experience may be substituted for degree. Education may be substituted for experience.</p>\n<p>In some instances, statutory or regulatory requirements may prohibit substitution of education and/or experience. </p>\n<p> </p>\n<p><strong><u>Physical, Environmental, and Special Working Conditions:</u></strong></p>\n<p>All County employees may be called upon to assist other departments in a declared emergency situation.</p>",
