Title of Job: Case Manager Date: June 16, 2026 Closing Date: July 2, 2026 Location: Toronto, ON - TAY Co-ops and Supportive Housing Compensation: $57,007 - $71,647 Annually Prorated Work Type: Onsite Job ID: 10850020 Existing or New Position: Existing Job Summary: As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues. The position attempts to empower the client, thereby increasing their independence, dignity and selfrespect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs. TAY (Transitional Aged Youth) Co-ops and Supportive Housing provides support to youth facing serious mental health challenges, who might also experience substance use challenges and/or homelessness. This program provides a range of supportive housing options across the city of Toronto Region either within LOFT owned houses or through head lease arrangements with private landlords. Each house is shared and comes fully furnished and with support services attached. TAY Co-ops and Supportive Housing follows a client-centered approach and the psychosocial rehabilitation recovery model – a treatment approach designed to teach emotional, cognitive, and social skills that help those diagnosed with a mental illness live and work in their communities as independently as possible. Community Support Workers and clients meet frequently to design and revise an individualized service plan based on the client’s goals and aspirations. Mental health counselling and harm reduction strategies are often discussed and provided as well. What You Will Do: Create individualized support and recovery plans with clients and professionals based on assessments. Support clients in building independence by teaching daily living skills (e.g., hygiene, medication, cooking, budgeting). Coordinate care by connecting clients with social, medical, and community support. Provide crisis intervention and help develop crisis plans. Represent the program professionally with clients, families, and community partners. Mediate conflicts and help clients learn problem-solving skills. Organize or support social activities to reduce isolation. Maintain accurate client records and documentation. Work collaboratively with the team, contribute ideas, and support new staff or volunteers. Provide on-call 24/7 crisis support on a rotating basis. Assist with preparing units for new clients. Provide case management and connect clients with services like housing and ID support. Help clients access financial supports such as Ontario Works (OW) and Ontario Disability Support Program (ODSP) or other financial resources as required. Other duties as assigned by Program Manager and/or designated supervisor. What You Bring: Post-secondary education in Social Services / Human Services or equivalent field. Minimum 3 years’ working experience with client population group. Understanding of challenges faced by Youths with mental illness, addictions, physical health concerns, and homelessness etc. Proficient in intake assessment, crisis intervention, advocacy, and problem-solving to support client needs. Ability to build therapeutic relationships and collaborate with clients on goals. Familiarity with community resources and professional services. Strong teamwork skills in a multidisciplinary environment. Strong interpersonal, communication, and counselling skills to effectively engage clients in sensitive areas such as life skills, healthy living, and relationships. Solid computer literacy for maintaining records, files, and reports. Effective analytical, organizational, and time management skills for coordinating client support and program activities. Demonstrates cultural competency, with an understanding of diverse backgrounds and inclusive practices. Must have a valid driver’s license, insurance, and a reliable vehicle to travel within the region. Work Environment: Staff will be required to work in clients’ rooms and may therefore be exposed to secondhand smoke. May also be required to be on-call as emergency contact for after hours. Frequent travel within the Central LHIN will also be required. Occasional evening and weekend hours may be required.
