The Rutland practice uniquely provides the community with access to Primary Care, Behavioral Health and Care Management services for the whole family. Primary Care services include on-site Behavioral Health providers, Social Workers, and Care Management, along with preventive programs like diabetes management and education. This position not only oversees clinical providers but also plays a strategic role in shaping organizational and community health initiatives.
