    "requisitionDescription": "<div><p><strong>Territory is Central/South County</strong></p><p><strong>12 pm - 10:30 pm- 4 shifts per week</strong></p><p><strong>$6,000 sign on bonus&nbsp;</strong></p><p data-pasted=\"true\">Under the general direction of the Director of Admissions and Palliative Care Services, the Admission Registered Nurse (RN) is responsible for determining the eligibility and preparation of patient/family need and the initial written plan of care. Position works closely with the Intake Coordinator, Clinical Service Department, and the Chief Operating Officer to ensure timely processing of all referrals. Position serves as a resource for questions/issues from the health care community and SFR staff. Position is full time and will include working weekends.</p><p><strong>REPORTS TO:&nbsp;</strong>Director of Admissions and Palliative Care Services<strong>&nbsp;</strong></p><p><strong>DIRECTLY SUPERVISES:&nbsp;</strong>None</p><p><strong>EDUCATION AND/OR TRAINING REQUIRED:</strong></p><p>Requires an Associate degree in nursing. Bachelor&rsquo;s or Certified Hospice and Palliative Care Nurse preferred.&nbsp;</p><p><strong>CERTIFICATIONS/LICENSES REQUIRED:</strong></p><p>Requires a valid current Florida Registered Nurse License and a valid Florida Driver&rsquo;s License with reliable transportation.</p><p><strong>EXPERIENCE/SPECIFIC SKILLS AND ABILITIES REQUIRED:</strong></p><p>Three years hospice experience <strong>preferred</strong>. Previous sales experience preferred. Ability to work flexible hours. Must understand the dynamics of human behavior and ability to communicate with patients and families in a crisis. Ability to communicate effectively with referral sources, and colleagues. Excellent oral and written communication, clinical and administrative skills. Able to prioritize varied projects. Dependable, flexible and tactful.&nbsp;</p><p><strong>PHYSICAL REQUIREMENTS:</strong></p><p>Ability to lift, push, pull twenty five <strong>(25)</strong> pounds without difficulty, work extended hours as required. Good visual and audio acuity. Ability to work under varying levels of stress.</p><p><strong>PRIMARY RESPONSIBILITIES:</strong></p><ol start=\"1\" type=\"1\"><li>Assists the Intake Coordinator with the coordination of referrals.</li><li>Meets the needs of the newly referred patient / families, completing the admissions process within 24 hours or sooner.</li><li>Gives a comprehensive explanation of hospice services, philosophies, and benefits addressing all components of care.</li><li>Obtains signatures on all necessary consents ensuring that patient / families are informed of their rights and responsibilities.</li><li>Performs a physical / environmental assessment, in addition to a psychosocial and spiritual assessment.</li><li>Provides primary clinical care and teaching, as needed, to stabilize the patient and facilitate the admissions process.</li><li>Documents assessments and establishes the initial plan of care and plan of treatment after consultation with another team member.</li><li>Is available and flexible with scheduling to meet the admission needs of the patient / families.</li><li>Is an active, enthusiastic member of the admissions team providing support for colleagues and creative ideas for improvement of the admissions process.&nbsp;</li><li>Maintains strict confidentiality and respect for patient / family privacy and possessions.&nbsp;</li><li>Provides emotional support to patient / family and other caregivers.</li><li>Increases knowledge of palliative care through active participation in educational opportunities.</li><li>Assists patient / family with understanding requirements during implementation of a disaster plan.</li><li>Participates in orientation and education of SFR personnel.</li><li>Participates in performance improvement and quality control activities.</li></ol></div>\n",
