Focuses on the unique needs of the rehab patient while transitioning them from an acute/post-acute care setting to a community setting. &nbsp;</p><p>&nbsp;</p><p>Rehabilitation nurses are part of a interdisciplinary team and coordinate care with therapists, case managers, social workers, and a Physiatrist (Physical Medicine and Rehabilitation Physician) to assess the unique rehab needs that address deficits, implement a rehab nursing plan of care with comprehensive interventions and develop ongoing caregiver and family education, &nbsp;They must demonstrates critical thinking to assure the delivery of safe, quality, age specific care while delegating effectively to appropriate team members.&nbsp;</p><p>&nbsp;</p><p>Rehab nurses are expected to document accurately and in a timely manner while utilizing the CARE tool as mandated by the Centers for Medicare/Medicaid Services. The rehab nurse is expected to adhere to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards.&nbsp;</p><p><strong>POSITION&rsquo;S ESSENTIAL RESPONSIBILITIES:&nbsp;</strong></p><p><strong>POSITION REQUIREMENTS:&nbsp;</strong></p><ul><li>Assesses/reassesses patient&rsquo;s condition based upon observation of patient and interview of patient and/or family, physician&rsquo;s diagnosis and data, diagnostic results, and relevant data from other healthcare providers. Assessment and reassessment of functional and cognitive deficits as it relates to transition to the community. &nbsp;&nbsp;</li><li>Uses professional knowledge to develop/revise nursing plans of care for assigned patients according to unit guidelines and criteria with expected outcomes. &nbsp;</li><li>Works with rehab team members to construct, develop, and implement rehabilitation treatment plan and rehabilitation plan of care to address emergent medical issues as well as functional/self-care/cognition deficits. &nbsp;&nbsp;</li><li>Delivers safe rehabilitation nursing interventions in an organized, efficient, and prioritized manner to address identified needs.</li><li>Attends weekly team conference to report on assigned patients&rsquo; status and progress toward rehab goals and medical status</li><li>Evaluates/re-evaluates patient&rsquo;s response to rehab nursing interventions and patient&rsquo;s progress to expected outcome(s) and community discharges.</li><li>Collaborates and communicates effectively with team members, physicians, and other healthcare providers to coordinate the medical and nursing plans of care.</li><li>Accepts delegated responsibilities for functions such as, but not limited to, charge nurse for unit activities on assigned shift.</li><li>Delegates responsibilities to appropriate personnel.</li><li>Documents all aspects of patient care, maintains patient records and care plans in a timely manner as appropriate for unit including scoring the CARE tool and providing data for the quality indicators as mandated by CMS.</li><li>Accurately complete Section GG scoring at admission, discharge, and per IRF-PAI guidelines.</li><li>Provides patients and family members with relevant health care education and instructions regarding treatments and procedures including pre/post hospital care as appropriate.</li><li>Assist in the orientation and training of new and tenured personnel, reassigned nurses, student nurses, Patient Care Assistants, and faculty; serves as a preceptor when requested.&nbsp;</li><li>Maintains self-control under stressful situations and workload pressures without exhibiting negative behaviors.&nbsp;</li><li>Demonstrates knowledge of disaster/emergency procedures and responds appropriately.</li><li>Assumes responsibility and accountability for individual knowledge, skills, performance and behavior in accordance with hospital, division, and unit standards of care and policies and procedures.&nbsp;</li><li>Utilizes measures to promote and maintain patient, visitor and personnel safety.</li><li>Maintains patient, employee, physician, and organization confidentiality; respects the rights, privacy, and property of others.</li><li>Demonstrates the appropriate use of infection control principles and personal protection equipment.</li><li>Supports the mission, values, and vision of the organization.&nbsp;</li><li>Demonstrates a positive attitude and takes the initiative to implement changes.&nbsp;</li><li>Operates as a team member planning, coordinating, developing, and implementing a rehab team care plan.</li></ul><p>&nbsp;</p><p><strong>ADDITIONAL RESPONSIBILITIES</strong></p><ul><li>Floats to assigned unit(s) as needed.</li><li>Performs tasks that are supportive in nature to the essential functions of the job.&nbsp;</li><li>Participates in staff meetings, educational programs, committees, QI activities and mandatory in-services.&nbsp;</li><li>Participates in maintaining unit functions.</li><li>Demonstrates initiative in personal/ professional development.</li><li>Presents a professional image.</li><li>Assists with control of costs through the judicious use of human and material resources.</li><li>Other duties as assigned.&nbsp;</li></ul><p><strong>&nbsp;</strong></p><p><strong>Required Skills/Abilities:&nbsp;</strong></p><ul><li>Current knowledge of the nursing role and scope of practice as defined by the State of Texas Nurse Practice Act and the Texas Board of Nursing.</li><li>Current knowledge of American Nurses Association (ANA) Code of Ethics.</li><li>Knowledge of current trends and demonstration of standards of care in assigned unit.</li><li>Successful completion of mandatory requirements including unit proficiencies/competencies.</li><li>Demonstration of effective problem solving, decision-making, interpersonal, and teamwork skills.</li><li>Demonstration of effective verbal and written communication.</li><li>Demonstration of positive customer service.</li></ul><p><strong>Qualifications:</strong></p><ul><li>Graduate of an accredited program for nursing education, BSN preferred.</li><li>Current licensure to practice professional nursing in the State of Texas.</li><li>Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.</li><li>Certification in specialty is preferred;
