    "requisitionDescription": "<div><div><div><p><br></p><p id=\"isPasted\"><strong>This is an onsite position where the Community Health Worker will be supporting our Pediatrician Team.</strong></p><p>Position Summary: The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services as part of the population health team. &nbsp;The Community Health Worker will support the Care Management Team and the Population Health Team.</p><p><strong>Program Specific&nbsp;Essential Duties and Responsibilities</strong></p><ul type=\"disc\"><li>Supports the care team with patient follow-up, reminders, and addresses barriers to care.&nbsp;</li><li>Document all services as required and will assist in meeting all program requirements.</li><li>Collects and analyzes data related to the project and works with data management staff for data collection.</li><li>Provides outreach and meets with patients to explain services and coordinates the treatment plan.</li><li>Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools as applicable.</li><li>Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.</li><li>Strong organizational and communication skills to act as a hub of information across multiple departments.</li><li>Any and all other duties as assigned.</li></ul><p>&nbsp;&nbsp;</p><p><strong>Qualifications and Education Requirements</strong></p><p>One of the following combinations of education and employment experience must be met in order to be considered for the position:</p><table border=\"0\" cellspacing=\"0\" cellpadding=\"0\"><tbody><tr><td valign=\"top\" style=\"width: 48.4848%;\"><p><strong>Education</strong></p></td><td valign=\"top\" style=\"width: 7.07071%;\"><p><strong>And</strong></p></td><td valign=\"top\" style=\"width: 44.4444%;\"><p><strong>Experience</strong></p></td></tr><tr><td valign=\"top\" style=\"width: 48.4848%;\"><p>High school diploma or GED required; Bachelor&rsquo;s degree is preferred. Preferred candidate will have Community Health Worker or Medical Interpreter certificate of completion.</p></td><td valign=\"top\" style=\"width: 7.07071%;\"><p>&nbsp;</p></td><td valign=\"top\" style=\"width: 44.4444%;\"><p>Minimum of two years relevant work or college-level internship experience required.
