Description

Due to continued growth and our commitment to improving patient access to care, we are excited to expand our team by adding two Referral Coordinators. In this vital role, you will serve as a key connection between our patients, providers, and specialty partners—helping ensure that every patient receives timely, coordinated care. The ideal candidate is highly organized, detail-oriented, and passionate about supporting patients as they navigate their healthcare journey. If you thrive in a collaborative environment and take pride in making the referral process smooth and efficient for both patients and providers, we invite you to join our dedicated care team Duties and Responsibilities Coordinate and process referrals Coordinate and process hospital discharges Submit and track authorizations Interface with corporate management, administrative personnel, medical, dental and behavioral staff Identifies the health needs of the patient and the expectations of the family. Makes an assessment of the resources and support network at the patient’s disposal. Assists in development, implementation, and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan; and monitors progress toward treatment goals. Maintains an active caseload, providing interventions as needed and within area of expertise and limits of credentials; assures procurement of additional services as needed. Reviews and facilitates, if appropriate, mental health referrals from other sources such as primary care, school health, etc. Offers alternative recommendations where necessary. Maintains patient and program records in accordance with applicable standards and regulations, grant requirements, etc. Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature. Participates in continuing education activities, remaining knowledgeable in area(s) of expertise. Attend meetings, as appropriate, and meet regularly with supervisor to exchange pertinent information and receive supervision. Participates in all safety programs, which may include assignment to an emergency response team. Other responsibilities as assigned to create a smoothly run efficient Clinic. Education High School Diploma or equivalent (required). Associate's degree in health administration, Medical Office Administration, or a related field (preferred). Certification in medical office administration or a related certification (e.g., Certified Medical Assistant, Referral Coordinator certification) is a plus. Minimum of 1-2 years of experience in a healthcare setting, preferably in a referral coordination, medical office, or patient services role. Experience with insurance verification and authorization processes is highly desirable. Familiarity with electronic health record (EHR) systems and scheduling software. Knowledge of medical terminology, patient referral processes, and healthcare regulations, including HIPAA compliance. Experience working in a Federally Qualified Health Center (FQHC) or community health setting is a plus.
