You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.</p><p><br></p><p><strong>Schedule:</strong> Monday though Friday, 8 - 4:30 PM, no nights, on-call, or weekends!&nbsp;</p><p><br></p><p>&nbsp;<strong>Benefits:</strong></p><ul><li>Up to $15k recruitment incentive for highly qualified candidates</li><li>Annual bonus plan</li><li>401K and company match</li><li>CME reimbursement and time off</li><li>Generous time off package!
