    "ExternalDescriptionStr": "<p style=\"margin-left: 0in;\"><strong>Job Summary</strong></p><p style=\"margin-left: 0in;\">The Care Coordinator II is responsible for coordinating multiple aspects of the youth’s and/or families’ care. This role provides the coordination and delivery of high-quality psychosocial care to youth and families. The Care Coordinator II works with other care providers and community partners to ensure that the overall needs of the client(s) are met.&nbsp;The Care Coordinator II may also be assigned to lead projects or mentor program staff.</p><p style=\"margin-left: 0in;\">This is a full-time (40 hours) position. Primarily on a Monday-Friday day shift schedule, but requires flexibility for evening/weekend hours based on client need. Additionally, the hire will need to be on the on-call rotation after the initial training period. Based in Monroe County at our Mustard St office in Rochester, NY but may require cross-county travel. Note that use of one's personal vehicle is required, and that mileage is eligible for a generous reimbursement.</p><p style=\"margin-left: 0in;\"><strong>Essential Job functions</strong></p><ul><li>Coordinate multiple aspects of the youth’s and/or families’ care.</li><li>Assess the youth and/or family’s needs and strengths while continuing to support development of the individual service plan in collaboration with the youth and family(ies), and/or multi-disciplinary treatment team.</li><li>Implement strength-based service plan including community based visits/home visits and assessments, identifying and coordinating collateral resources, recognizing gaps and needs, assessing results, and communicating with the multi-disciplinary treatment team.</li><li>Utilize experience, education, age-specific knowledge and cultural competence in order to assess and support youth and/or families to achieve positive outcomes.</li><li>Coordinate and deliver psychoeducational training for youth and/or families and groups in a variety of settings based on identified needs.</li><li>Meet&nbsp;with youth individually to assess and teach skill development and report on their progress.</li><li>May organize and facilitate skill-based groups for youth.</li><li>Develop, maintain and update all required documentation in a timely manner utilizing effective written and oral communication.</li><li>Provide or arrange for transportation of youth and/or families to and from medical, behavioral health, or other treatment related appointments as needed.</li><li>May be assigned to lead projects or mentor program staff.</li><li>On-call responsibilities for <strong>Foster Care</strong> and Youth ACT programs.</li><li>While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.<br>&nbsp;</li></ul><p style=\"margin-left: 0in;\"><strong>Education &amp; Experience</strong></p><ul><li>Bachelor’s degree required.</li><li>Minimum 18 months of relevant experience required.</li><li><i><strong>Therapeutic Foster Care</strong> program:</i> Master's degree in Social Work or a related human services field, OR Bachelor's degree in Social Work or related human services field with 2 years of related experience.<br>&nbsp;</li></ul><p style=\"margin-left: 0in;\"><strong>SPECIAL REQUIREMENTS</strong></p><ul><li><p>Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.&nbsp;</p><p><br>&nbsp;</p></li></ul><p style=\"margin-left: 0in;\"><strong>Knowledge, Skills &amp; Abilities</strong></p><p>In addition to demonstrating the <i>Hillside Professional Competencies</i> of Communicates Effectively, Personal Excellence, Cultural Competence, Builds and Leverages Relationships, and Optimizes Decision Making, the following occupational competencies must be demonstrated:</p><ul><li>Ability to understand and adhere to all federal, state, and local statutes, regulatory agency standards and Hillside policies&nbsp;</li><li>Ability to de-escalate and manage crisis situations both in-person and by phone</li><li>Ability to deliver psychoeducational training</li><li>Ability to work collaboratively as part of a multi-disciplinary treatment team</li><li>Ability to work through a series of issues using independent judgment</li><li>Ability to be independent and self-directed<br>&nbsp;</li></ul><p style=\"margin-left: 0in;\"><strong>Physical Demands &amp; Work Environment</strong></p><ul><li>Must be able to work a variable scheduling including evenings and weekends</li><li>The following daily physical demands are required:<ul><li>Sitting (up to 6 hours)</li><li>Occasional standing (up to 2 hour)</li><li>Occasional walking (up to 2.5 hours)</li><li>Driving (several times a week up to 6 hours)</li><li>Continuous balancing (up to 8 hours)</li><li>Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)</li><li>Weekly lifting up to 10-15 lbs. on a daily basis</li><li>Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)</li><li>Occasional exposure to dust, fumes, gases, chemicals, or smoke is apparent&nbsp;&nbsp;</li><li>Ability to change positions as needed<br>&nbsp;</li></ul></li></ul><p style=\"margin-left: 0in;\"><strong>SPECIAL CONSIDERATIONS</strong></p><p><span style=\"font-family: Calibri, sans-serif;\"><span style=\"font-size: 11pt;\">While Hillside is a restraint free environment, there may be times in a “life or limb” situation, that staff may be required to physically restrain clients weighing between 50 lbs. and 300 lbs. and guide them safely to the floor.
