Description
The role of the Sherwood Park – Strathcona County PCN Registered Nurse (RN) is to work collaboratively with the patients, family physicians, other team members to provide comprehensive clinical services as identified for the patients of the PCN.
The RN will assess patients, develop, implement and evaluate care plans for patients and provide indirect support to patients. The RN will work to full scope of practice with an emphasis on providing comprehensive care, education and guidance to patients with chronic disease. 0.8 FTE Temporary until August 31, 2027 Schedule: MTW & F 9:00AM - 5:15PM Pay range, based on experience $40.98 - $53.96 Paid vacation and sick time Eligible for Health Benefits Health & Wellness Spending Account 9% RSP matching program Education support, including funding and paid time for professional development
KEY RESPONSIBILITIES
Include, but are not limited to the following:
General Assessment of patient conditions, needs and goals in collaboration with the patient, attending physician and interdisciplinary team Identify needs and set priorities in collaboration with the client and other team members Provide short term interventions as needed Facilitate appropriate referrals to community resources and programs Provide relevant patient education to prevent and manage clinical conditions Participate in evaluation of the clinic and its services Assist in planning, developing, and monitoring for services provided To assist in the development of policies and protocols for chronic disease management, geriatric care, maternal/child and mental health To develop systems and communications protocols to facilitate patient follow up To assist in setting up health promotion programs relevant to the needs of the practice population. To assist in developing and implementing patient self-management programs. Assist in the implementation of strategies for improving the quality and provision of care for the practice population, Any other duties that may be reasonably required for the success of the PCN.
EDUCATION & EXPERIENCE Baccalaureate degree in Nursing preferred Five years or more of related nursing experience, preferred Current CRNA Registration, standard BLS provider certificate Initiative, accountability, decision-making, interpersonal, and communication skills, and Demonstrated ability and willingness to work in a dynamic environment that requires flexibility and balancing of competing priorities Experience in PAP testing Experience working in a team setting with other health professionals, Advanced skills in caring for patients with chronic, complex, and/or specialized needs Experience in establishing protocols for program startups, especially developing plans and priorities when workload varies
SKILLS & ABILITIES Expert knowledge of nursing processes and practices including intervention strategies and therapy Strong, effective written and oral communication skills, high level of perceptiveness, Critical thinking skills Ability to build a good working relationship with healthcare professionals, clients and other members of the community Ability to prioritize, manage time effectively and be flexible in a very active work environment. Respect and understanding of confidentiality, A strong knowledge base in medications and is able to conduct medication reviews with patients Customer service orientation, Thrives in a complex changing environment, High levels of integrity and professional behavior At least basic computer skills in word processing and email. Knowledge of, and ability to use, electronic medical records

The RN will work to full scope of practice with an emphasis on providing comprehensive care, education and guidance to patients with chronic disease.</span></span></span><ul><li><strong>0.8 FTE in the development of policies and protocols for chronic disease management, geriatric care, maternal/child and mental health</span></span></span></li><li><span style=\"font-size:12pt;\"><span style=\"line-height:115%;\"><span style=\"font-family:Aptos, sans-serif;\">To develop systems and communications protocols to facilitate patient follow up</span></span></span></li><li><span style=\"font-size:12pt;\"><span style=\"line-height:115%;\"><span style=\"font-family:Aptos, sans-serif;\">Baccalaureate degree in Nursing preferred</span></span></span></li><li><span style=\"font-size:12pt;\"><span style=\"line-height:115%;\"><span style=\"font-family:Aptos, sans-serif;\">Five years or more of related nursing experience, preferred </span></span></span></li><li><span style=\"font-size:12pt;\"><span style=\"line-height:115%;\"><span style=\"font-family:Aptos, sans-serif;\">Current CRNA Registration, standard BLS provider certificate</span></span></span></li><li><span style=\"font-size:12pt;\"><span style=\"line-height:115%;\"><span style=\"font-family:Aptos, sans-serif;\">Initiative, accountability, decision-making,
