By joining our team, you will share in the joy of providing “care with dignity and life with hope.”</span></p>\n<p><span style=\"font-family: 'Arial',sans-serif; font-size: 11.0pt\"><span style=\"text-decoration: none\"> </span></span></p>\n<p><em><span style=\"font-family: 'Arial',sans-serif; font-size: 11.0pt\">Responsibilities:</span></em></p>\n<p><span style=\"font-family: Wingdings; font-size: 11.0pt\"><span>Ø  </span></span><span style=\"font-family: 'Arial',sans-serif; font-size: 11.0pt\">Utilize life experience to engage people, within the program and in the community, developing brief person-centered service plans and providing referrals to needed services.<span>  </span></span></p>\n<p><span style=\"font-family: Wingdings; font-size: 11.0pt\"><span>Ø  </span></span><span style=\"font-family: 'Arial',sans-serif; font-size: 11.0pt\">Assist counselors with referring clients to alternative levels of care when clinically indicated. </span></p>\n<p><span style=\"font-family: Wingdings; font-size: 11.0pt\"><span>Ø  </span></span><span style=\"font-family: 'Arial',sans-serif; font-size: 11.0pt\">Work collaboratively with Hospital Screening Brief Intervention and Referral Treatment Health Coaches to assess individuals’ needs and connect them to treatment and other support services across Suffolk County. </span></p>\n<p><span style=\"font-family: Wingdings; font-size: 11.0pt\"><span>Ø  </span></span><span style=\"font-family: 'Arial',sans-serif; font-size: 11.0pt\">Remain actively involved and follow up with individuals and/or families after their hospital discharge.</span></p>\n<p><span style=\"font-family: Wingdings; font-size: 11.0pt\"><span>Ø  </span></span><span style=\"font-family: 'Arial',sans-serif; font-size: 11.0pt\">Promote the Program through information, education and outreach to local communities, agencies and legal/medical professionals. Participate in community events, such as Narcan Trainings, Fairs and Forums.</span></p>\n<p><span style=\"font-family: Wingdings; font-size: 11.0pt\"><span>Ø  </span></span><span style=\"font-family: 'Arial',sans-serif; font-size: 11.0pt\">Collect data to assist reporting to funding sources.
