ummary
The Licensed Practical Nurse (LPN) provides care to designated patients under the direct supervision of the Registered Nurse.
Essential Functions
Clinical Practice :
Observes and collects patient and family data demonstrating knowledge of variance from age-based norms. Uses appropriate care guidelines; policies, procedures and standards in daily care assignment. Communicates pertinent patient information/data to the RN on an ongoing and timely basis. Provides care related to personal hygiene, activities of daily living, and comfort measures. Completes patient care as assigned by the RN. Gathers data related to patient response to assigned interventions and reports to RN as appropriate. Provides care safely with adequate quality and timeliness according to applicable standards and appropriate to the age and development level of patients.
Patient/Family Education :
Collects data related to patient education. Distributes educational materials per standard of care. Reinforces patient education. Follows plan for patient/family education assigned by RN. Apply a minimum of one health literacy method when communicating with patients/clients.
Continuum of Care Management :
Completes assigned tasks that facilitate discharge and transfer planning.
Patient/Customer Satisfaction :
Gathers data about patient/family service expectations and reports to RN. Understands unit performance and incorporates feedback into care and unit operations.
Resource Utilization & Management :
Uses supplies and equipment to achieve cost-effective care. Utilizes appropriate standards of care, order sets, and pathways to achieve patient outcomes in a cost-effective manner.
Performance Assessment & Improvement :
Participates in actions to achieve the care center performance assessment and improvement plan. Proposes improvements for unit operations and quality clinical outcomes.
Leadership :
Uses effective interpersonal skills. Communicates appropriate data related to patients and unit operations to the RN.
Continuous Learning :
Participates in pertinent annual training, education and development activities to improve knowledge and skills in order to contribute to the organizational goal of continuous learning.
HealthNet Accountabilities :
Schedules surgery including precertification scheduling, pre-op training and instruction to patients. Coordinates office schedules of providers. Performs surgical procedural assistance as needed. Provides functional guidance to office staff which support providers. Coordinates provider and midwifery site accommodation practices at OB Care Center.
