    "requisitionDescription": "<div><p>&nbsp;&nbsp;&nbsp;&nbsp;<span style=\"font-size: 12pt;font-family: Arial, sans-serif;\">The Geriatrics Workforce Enhancement Program</span><span style=\"color: black;background-color: white;font-size: 12pt;font-family: Arial, sans-serif;\"> is designed to </span><span style=\"color: rgb(10,10,10);background-color: rgb(254,254,254);font-size: 12pt;font-family: Arial, sans-serif;\">improve health outcomes for older<br>  adults by developing a healthcare workforce that maximizes patient and family<br>  engagement, and by integrating geriatrics and primary care. </span><span style=\"color: black;background-color: white;font-size: 12pt;font-family: Arial, sans-serif;\">The program </span><span style=\"font-size: 12pt;font-family: Arial, sans-serif;\">will focus on<br>  improving screening for common geriatric problems, with an initial focus on<br>  improving advance care planning, depression screening and colorectal cancer<br>  screening</span><span style=\"color: black;background-color: white;font-size: 12pt;font-family: Arial, sans-serif;\">. As a Community Health<br>  Coach, your job would be to offer support and coaching of positive, healthy<br>  behaviors to older adults enrolled in the Geriatric Workforce<br>  Enhancement Program. Working alongside a health care team, you would work to<br>  lower the negative health outcomes in Indiana, and more specifically in<br>  Marion County and Central Indiana.  </span>&nbsp;&nbsp;&nbsp;</p>\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</p>\n<p><span style=\"background-color: rgb(204,204,204);font-size: 12pt;font-family: Arial, sans-serif;\">Essential Functions</span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</p>\n<p><span style=\"background-color: rgb(238,238,238);font-size: 12pt;font-family: Arial, sans-serif;\">Patient Treatment:</span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</p>\n<p><span style=\"background-color: white;font-size: 12pt;font-family: Arial, sans-serif;\">Provides direct crisis<br>      intervention, short-term counseling, information and referral, and<br>      provides psychosocial assessment as necessary. Collaboratively plans and<br>      facilitates appropriate interventions that will improve overall family health<br>      and well-being. Serves as a liaison between the patient and the health<br>      care provider concerning the broad psychosocial aspects of the patient's<br>      course of treatment.
