With three (3) conveniently located health centers, three (3) behavioral health sites and a friendly welcoming staff, PHC aspires to serve all the health care and wellness needs of the entire family.</p><p><strong>About the Role:</strong></p><p data-start=\"188\" data-end=\"590\" data-pasted=\"true\">The Outreach Nurse &ndash; Mobile Health &amp; Community Engagement is responsible for delivering clinical care, preventive screenings, and health education in community-based settings to improve access to care for underserved populations. This role integrates nursing practice, population health, and patient navigation to support care gap closure, chronic disease management, and preventive health initiatives.</p><p data-start=\"592\" data-end=\"963\">The Outreach Nurse ensures that all outreach services and education align with organizational capabilities and are based on identified community health needs in accordance with AAAHC and HRSA standards. This position plays a key role in connecting community members to primary care services and converting outreach encounters into scheduled and completed clinical visits.&nbsp;</p><p data-start=\"592\" data-end=\"963\"><strong>Duties and Responsibilities:</strong></p><ul data-start=\"1007\" data-end=\"2539\"><li data-section-id=\"1p7nzb6\" data-start=\"1007\" data-end=\"1168\"><p data-start=\"1009\" data-end=\"1168\">Provide clinical services in community and mobile health settings, including vital signs, screenings, vaccinations, and basic triage within scope of practice</p></li><li data-section-id=\"ng5h2b\" data-start=\"1169\" data-end=\"1317\"><p data-start=\"1171\" data-end=\"1317\">Deliver culturally competent, evidence-based health education related to chronic disease
