This role provides patient centric care and is primarily responsible for nursing assessment and diagnosis, planning, implementation, and evaluation.</p>\n<p> </p>\n<p>This position is 0.95 FTE, 38 hours/week.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Assesses physical, emotional, social, and spiritual needs and evaluates patients’ adaptation to health changes</li>\n <li>Develops a plan of care based on patients’ disease, symptoms, and response to treatment</li>\n <li>Educates patients, families, and caregivers on disease processes, treatment, side effects and symptom management</li>\n <li>Coordinates with inter-disciplinary team to ensure continuity of care</li>\n <li>Refers patients, families and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc.</li>\n <li>Implements, evaluates and modifies plan of care</li>\n <li>Integrates research and evidence based knowledge into clinical practice</li>\n</ul>\n<p>Care Coordination:</p>\n<ul>\n <li>Performs comprehensive initial assessment of patient, family and caregiver</li>\n <li>Performs ongoing assessment of patient, family and caregiver as patient progresses through Transplant process</li>\n <li>Monitors patient response to therapies, including lab values and symptom assessment, in collaboration with the medical, nutrition and pharmacy staff</li>\n <li>Provides psychosocial support to patients and families</li>\n <li>Functions as resource for rotating physician staff regarding standard practice and operations in the clinic</li>\n <li>Reviews all orders written by the medical staff</li>\n <li>Communicates hospital admissions to the Charge RN</li>\n <li>Coordinate home infusions provided by outside Home Infusion agencies after initial referral by Transition RNs</li>\n <li>Provide continuity of nursing care by communicating effectively with Charge, Evening, Weekend, Infusion</li>\n <li>Room, Procedure, Triage, and Inpatient RN’s</li>\n <li>Documents all assessments, teaching, response to therapies and plans of care in a timely manner</li>\n <li>Keeps patient Kardex up to date</li>\n <li>Functions as primary telephone contact for patients and their caregivers</li>\n <li>Participates with all disciplines in daily rounds to plan and coordinate patient care</li>\n</ul>\n<p>Patient and Caregiver Teaching:</p>\n<ul>\n <li>Provides initial education and Clinic orientation. Reviews Patient Resource Manual with patient and family at arrival visit</li>\n <li>Assumes primary responsibility for chemotherapy teaching with patients and caregivers</li>\n <li>Assumes primary responsibility for patient education regarding medications and therapies, including home infusions</li>\n <li>Assumes primary responsibility for patient education regarding symptom management</li>\n <li>Assumes primary responsibility for departure teaching when patient is leaving the Acute Transplant Program</li>\n</ul>\n<p>Support for Research:</p>\n<ul>\n <li>Acts as patient advocate in coordination of research consent forms</li>\n <li>Assists in obtaining research data in conjunction with patient care data</li>\n <li>Maintains a working knowledge of current protocols related to the patient population with whom they work. Knows research resources and how to access</li>\n</ul>\n<p>Communication and Teamwork:</p>\n<ul>\n <li>Participates in Committees and Task Forces to review and improve current standards</li>\n <li>Serves as a resource to other nurses caring for patients undergoing BMT transplant</li>\n <li>Flexible in incorporating patient care outside own team assignment to assist with patient care needs in other areas of clinic</li>\n <li>Informs Nurse Manager of high-risk patient situations or operational issues</li>\n <li>Communicates in a respectful and effective manner with other staff</li>\n <li>Demonstrates appropriate problem solving when working with other departments</li>\n</ul>\n<p>Maintaining And Developing Own Skills:</p>\n<ul>\n <li>Participates in continuing education programs</li>\n <li>Keeps current in area of expertise through journals, books, and conferences</li>\n <li>Regular attendance and punctuality are essential functions of this job in order to carry out the primary responsibilities.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Required:</strong></p>\n<ul>\n <li>Graduate from an accredited school of nursing</li>\n <li>Must have current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL); and MSL primary state of residence must be updated to WA within 60 days of hire. This requirement may be waived under specific circumstances, subject to approval.</li>\n <li>Applicants that do not have previous oncology experience will be provided additional training and support</li>\n <li>Current BLS AHA Health Provider Card (or equivalent course, such as by the American Red Cross) and renewal required every two years</li>\n <li>Additional certification may be required dependent upon department. </li>\n <li>Critical thinking; the ability to practice the cognitive skills of analyzing, applying standards, discriminating, information seeking, logical reasoning, predicting, and transforming knowledge</li>\n <li>Ability to develop clinical judgment.</li>\n <li>Time Management skills; the ability to organize and manage time and tasks independently</li>\n <li>Ability to communicate effectively by looking and listening for cues, asking open-ended questions, exploring cues, using pauses, screening responses, and clarifying response.</li>\n <li>Ability to effectively listen by using reflecting, acknowledging, summarizing, empathizing, and paraphrasing skill.</li>\n <li>Ability to effectively provide information by checking what information the person knows already, giving small amounts of information at a time, using clear terms and avoiding jargon, avoiding detail unless it is requested, checking for understanding, and pausing and waiting for a response.</li>\n</ul>\n<p><strong>Preferred:</strong></p>\n<ul>\n <li>Bachelor of Science in Nursing</li>\n <li>Two years of nursing experience preferred.</li>\n</ul>The hourly pay range for this position is from $47.64 to $88.19 and pay offered will be based on experience and qualifications.  Registered Nurses are paid on a step-scale basis and are also eligible for shift differentials and other pay premiums such as advanced degree, preceptor and certification.   Although Fred Hutch is not sponsoring most H-1B visas at this time, candidates who already hold an H-1B sponsored by another organization and are currently in the U.S. may be eligible for this position. This position may be eligible for relocation assistance.This position is 0.95 FTE, 38 hours/week.
