Key responsibilities include: Wellness & Prevention Lead wellness programming and health education Promote healthy aging and chronic condition management Conduct assessments to support overall well-being Supporting Independence Connect residents with resources and services that help them remain in their apartments Recommend safety adaptations and assistive supports Care Coordination Coordinate weekly on-site physician clinic visits Assist with lab orders and physician communication Maintain accurate resident records, forms, and emergency contact information Collaboration & Transitions Partner with the Healthcare Navigator to support residents and families Guide transitions to higher levels of care when needed
