Description

St. Francis Center (SFC), an institution of the Episcopal Diocese of Colorado, was established in 1983 .  SFC is a refuge for people experiencing homelessness in the metro Denver area. Our programs and services are tailored toward adults and focus on daytime shelter, social services, health and wellness, housing, employment, outreach , and a new program to provide non-congregate shelter operations for people transitioning out of homelessness . In SFC’s services, the people we serve can work with a variety of specialists, depending on their individual needs, to establish the first step toward transformation, new beginnings , and stability. Department: Housing and Supportive Services Exemption Status: Full-time, exempt Location of work : This program is located at a privately owned hotel, the Comfort Inn (4685 Quebec Street), a 136-unit hotel being leased, and services funded by the City of Denver’s Office of Supportive Housing. We operate 24/7, 365 days a year, rain or shine. Reliable transportation is a must . POSITION OVERVIEW Under the general supervision of the NCS Director , you will support guests struggling with complex medical, psychiatric, substance mis use, and other psychosocial stressors as they work towards permanent housing solutions. You will provide intensive clinical case manag psychoeducation, crisis intervention , and use various assessment tools to support successful outcomes for guests. You will also advocate and coordinate care on an on-going basis with other partner agencies, families, and guests. Responsibilities: Evaluate and address social, emotional, and physical needs of gues ts by applying evidence-based assessments . Help guests learn coping strategies to manage their self- identified issues. Collaborate with multi disciplinary health care teams to treat and plan for the psychosocial and medical needs of guests . D evelop and implement stability plans and complete required reporting activities by specific timeframes . Provide supportive counseling or crisis intervention to guests . Provide external referral s when clinically appropriate . Provide education, information , and referrals to gues ts regarding mental health, substance mis use , and related services. Advocate on behalf of guests , with partner agencies, medical practitioners , and within the community. Schedule room visits when necessary to assess the living situations of guests for accessibility , appropriate service referrals , and to determine the need for further services. Plan, coordinate, and lead prosocial activities for guests in co llaboration with the supportive services team . Organize and lead psychoeducational groups, with a focus on harm reduction. Work with the management team in program development as requested . Provide guest care in a nondiscriminatory and non j udgmental way. EDUCATION A ND EXPERIENCE (MINIMUM REQUIREMENTS) Master’s degree in social work or related field. Lice nsed or licensed eligible (LCSW , LPC , or L icensed Psychologist ) . Three (3) years of experience working with individuals experiencing chronic homelessness in a community setting. Working knowledge of m etro Denver agencies, programs, criminal justice system, veterans’ services , and homeless services. Experience as a service provider with a multidisciplinary health care team. Work experience with individuals experiencing substance misuse. Interest and experience in working with a harm reduction model. Trauma-informed approach or willingness to learn . Ability to work independently.
