    "requisitionDescription": "<div><h2 id=\"isPasted\">Job Overview</h2><p>&nbsp; &nbsp;&nbsp;</p><p>This is a union position.</p><p>Provides medical support services to behavioral health programs in the following areas: medication management support and education, assistance to prescribing providers, and support to daily treatment operations. Our LVNs utilize medical expertise to provide services and support as part of an integrated treatment team, and to assist clients in their recovery. LVNs work in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360&rsquo;s philosophy, goals, policies, mission, and vision. The LVN is customer service focused and is quick to take the lead in providing crisis intervention and participant stabilization. The LVN has a high degree of self-awareness and the ability to stay calm under pressure.</p><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</p><h2>Key Responsibilities</h2><p>&nbsp; &nbsp;&nbsp;</p><p>General Responsibilities:</p><ul><li>Assist prescribing providers in the day-to-day monitoring, treatment, and maintenance of client medication needs as qualified and licensed.</li><li>Administratively, takes the lead with the following: MAR binders, client appointments, and assisting with ordered labs and blood draws.</li><li>Effectively communicates with MSS team, site leadership, and program staff.</li><li>Facilitates health, wellness, and medical education.&nbsp;</li><li>Establish working relationships with community partners and primary care providers in order to provide for high-quality continuity of care.</li><li>Responsible for tracking and ordering medical supplies.</li><li>Develop and assess effectiveness of individualized treatment plans and consumer progress.</li><li>Serve as medical care coordinator for medication clients, and complete of all coordinated care documentation.</li><li>Assist clients to develop independent living skills that promote independence and self-sufficiency.</li><li>Arrange work schedule in accordance with agency&rsquo;s needs.</li><li>Comply with the agency&rsquo;s policies and procedures.</li></ul><p>Perform other duties as assigned. Documentation:</p><ul><li>Conduct chart audits and report findings to Medical Management and Nurses.</li><li>Ensures that MARs are complete and accurate.</li><li>Submit contractual reporting and documentation requirements, by law or funding agency requirements, in a timely manner.</li><li>Maintain documentation in compliance with agency, HIPAA and DMC/SAPC standards.</li><li>Must meet productivity benchmarks as outlined by medical and clinical directors.</li><li>Meet expected performance standards as assigned by Supervisor.</li><li>Monitor and document for medical quality assurance.</li><li>When providing direct services, complete all required paperwork, charting, records maintenance in a timely manner.</li></ul><p>Administrative:</p><ul><li>Responsible for signing off medications, counting controlled medications, reading log entries for up-dates, and recording all changes.</li><li>Maintains stock of OTC medications and First Aid supplies.</li><li>Acts as liaison for removal of bio hazardous waste.</li><li>As needed, assists in supporting and monitoring office visits.</li></ul><p>Training, Supervision, and Quality Improvement:</p><ul><li>Actively participates in agency and team meetings.</li><li>Participates in training opportunities.</li><li>Communicates collaboratively with all members of the behavioral health team including: medical, mental health, substance use disorder, and other staff.</li><li>Completes all assigned trainings and Relias trainings in a timely manner.</li></ul><p>Professionalism and Other Duties:</p><ul><li>Responds to requests for coordination of facility needs from Supervisor and other Leadership.</li><li>Attend regular staff meetings and other meetings as assigned.</li><li>Reads emails daily and responds when appropriate.</li><li>Provide services to clients off-site and in the field as needed.</li><li>Serve as the primary contact for an assigned caseload of clients who only receive medication services, and ensure that assessments, treatment plans, and other required client record components are current and up to date.</li></ul><p>And, other duties as assigned.</p><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</p><h2>Education and Knowledge, Skills and Abilities</h2><p>&nbsp; &nbsp;&nbsp;</p><p>Education and Experience Required:</p><ul><li>Licensed vocational nurse in California.</li><li>IV certification &bull; Valid CA Driver License and automobile insurance.</li></ul><p>Required:</p><ul><li>Active LVN licensed.</li><li>Minimum of 1 years community based program management experience or equivalent.</li><li>Knowledge of trauma preferred.</li><li>Commitment to providing nonjudgmental quality health care to those in need.</li><li>Experience serving seriously and persistently mentally ill clients with co-occurring challenges.</li><li>Experience and commitment to working with diverse communities.</li></ul><p>Desired:</p><ul><li>Active LVN with minimum 3 years of experience.</li></ul><p>Background Clearance Required:</p><ul><li>Department of Justice background clearance.</li><li>Must not be on parole or probation for a minimum of two years.</li></ul><p>Knowledge Required:&nbsp;</p><ul><li>Knowledge issues of related to substance abuse, mental health, and criminal background.</li><li>Culturally competent and able to work with a diverse population.</li><li>Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.</li></ul><p>Skills and Abilities Required:</p><ul><li>Detail oriented.</li><li>Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.</li><li>Professionalism, punctuality, flexibility and reliability are imperative.</li><li>Excellent verbal, written, and interpersonal skills.</li><li>Integrity to handle sensitive information in a confidential manner.</li><li>Action oriented.</li><li>Strong problem-solving skills.</li><li>Excellent organization skills and ability to multitask and juggle multiple priorities.</li><li>Outstanding ability to follow-through with tasks.</li><li>Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility.</li><li>Strong initiative and enthusiasm and willingness to pitch in whenever needed.</li><li>Bilingual in fluent Spanish (read, write, speak).</li></ul><p>Other Requirements:</p><ul><li>First Aid Certified within 30 days of employment.</li><li>CPR Certified within 30 days of employment.</li><li>A valid California driver&rsquo;s license and current automobile insurance.</li></ul><p>Desired:</p><ul><li>Bilingual in English/Spanish.</li><li>Bachelor&rsquo;s Degree in related field.</li><li>One year of experience in a residential substance abuse treatment setting or a withdrawal management/detox setting.</li><li>Knowledge of co-occurring disorders and trauma informed treatment.</li><li>Knowledge of Clinical documentation (treatment plans, progress notes etc.).</li><li>Experience working with criminal justice population.</li></ul><p>We will consider for employment qualified applicants with arrest and conviction records.</p><p>In compliance with the California Department of Public Health&#39;s mandate, all employees must be able to provide proof of COVID-19 vaccination.
