Part-Time Charleroi, PA Location Overall Function: Implements both facility and community-based therapeutic care, training, and rehabilitation of adults with Intellectual Disabilities who attend Riverside Horizons. Essential Functions: Works as part of a team, while facility-based, to provide care and pursue desired activities for individuals present at the facility. Responsible for providing transportation, supervision, and care of 1-6 individuals in integrated community settings. Provides activities and supports for: Developing skills and competencies necessary to pursue competitive integrated employment. Participating in community activities, organizations, groups, associations, or clubs to develop social networks. Identifying and participating in activities that provide purpose and responsibility. Fine and gross motor development and mobility. Participating in community opportunities related to the development of hobbies or leisure/cultural interests or to promote personal health and wellness (e.g., yoga class, hiking group, walking group, etc.); Participating in community adult learning opportunities. Participating in volunteer opportunities. Opportunities focused on training and education for self-determination and self-advocacy. Learning to navigate the local community, including learning to use public and/or private transportation and other transportation options available in the local area. Developing and/or maintaining social networks and reciprocal relationships with members of the broader community (e.g., neighbors, coworkers, and other community members who do not have disabilities and who are not paid or unpaid caregivers) through natural opportunities and invitations that may occur. Assisting participants, caregivers, and providers with identifying and utilizing supports not funded through the waiver that are available from community service organizations, such as churches, schools, colleges/universities and other post secondary institutions, libraries, neighborhood associations, clubs, recreational entities, businesses, and community organizations focused on exchange of services (e.g., time banks); and Assisting participants and caregivers with providing mutual support to one another (through service/support exchange) and contributing to others in the community. Successfully completes training in Medication Administration and administers medications according to protocol. Maintains certification in CPR/AED and First Aid and initiates emergency procedures, as necessary. Maintains cleanliness and order in the facility and assists in preparing and serving food. Assists in developing and updating active goal plans based on ongoing assessments, evaluations, observations, and interviews. Completes service notes and all required documentation in a timely and accurate manner. Adheres to all policies, laws, regulations, and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures. Qualifications High School Diploma or GED. Ability to transport oneself daily to the facility in an insured vehicle. Must be available to work Monday through Friday-9:00 AM to 3:00 PM. Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program. Must have a valid Pennsylvania Driver's License and a safe driving record. Must successfully obtain Criminal History Clearance from the PA State Police and FBI Fingerprint Clearances. Subject to pre-employment and random or targeted drug testing. Ability to assist, move, and/or transfer individuals may be required. Ability to assist, move, and/or transfer program equipment and supplies weighing up to 30 pounds.
