Description

We are seeking a highly engaged and patient-focused Care Navigator/Field Support professional to support the In-Patient Case Management team. This role will report directly to the Director and Senior Manager of In-Patient Case Management and will serve as a key liaison between patients, providers, and case managers to help ensure continuity of care, timely coordination of services, and positive patient outcomes. The ideal candidate is compassionate, highly organized, adaptable in fast-paced healthcare environments, and committed to delivering exceptional patient-centered support both telephonically and in the field. You are: Compassionate with patients’ various needs Action-oriented with strong follow-through and problem-solving abilities Collaborative and able to build strong relationships with patients, providers, hospitals, and internal teams Resourceful and proactive in identifying barriers to care and escalating concerns appropriately Mission-driven and passionate about improving patient outcomes and experiences Organized and capable of managing multiple priorities in a fast-paced environment A strong communicator with excellent customer service and interpersonal skills Comfortable working independently in both office and field-based settings Care Plan, Education and Continuity of Care Support the Case Manager and other providers in the development of care plans for patients, including transitional plan arrangements for optimal patient care after an acute event Listen to patient concerns, identify and document care coordination barriers, and escalate issues to the appropriate Case Manager or provider Conduct patient outreach and follow-up calls to assess patient status, identify barriers to care, and reinforce treatment plans Ensure that the patients’ treatment requirements are met Provide patient instructions as outlined on care plan Ensure timely communication between patients, providers, facilities, and health plans regarding changes in patient condition or care needs Participate in interdisciplinary team huddles, case reviews, and care coordination meetings as assigned Assist Case Manager with patient follow-up activities and document progress Administrative Coordinate and schedule patient appointments, specialist referrals, diagnostic testing, treatments, consultations, and post-discharge follow-up visits Maintain accurate and timely documentation in accordance with departmental, regulatory, and organizational standards Perform analytical, data entry tasks, and clerical projects as assigned Prioritize and manage several responsibilities simultaneously in a timely fashion without compromising quality such as accuracy, neatness, and thoroughness. Conduct patient surveys to measure patient activation scores Prepare reports and provide administrative assistance as assigned Perform other tasks as assigned by supervisor 1+ years of Doctor’s office/clinic setting, CHW, MA, or CNA experience Proficient with Healthcare Management Systems and Microsoft Office Word and Excel Bilingual: Spanish/English, Mandarin/English, or another second language is strongly preferred Outstanding communication skills, both written and verbal Excellent organizational skills Ability to travel locally (visiting hospitals) Strong comfort with working in a fast-paced environment An authentic passion for improving the lives of underserved patients and advancing value-based care This is a field based position where the staff will need to visit our partnered hospitals located in the San Francisco area. When not conducting onsite visits, the role supports remote work. The total compensation target pay range for this role is $18.00 - $25.00 per hour. The salary range represents our national target range for this role. Actual compensation will be determined based on geographic location (current or future), experience, and other job-related factors.
