We believe education is the key that enables all people to create economic and civic opportunities for themselves, their families, and their community. &nbsp;ESH&rsquo;s Adult &amp; Workforce Programs align with our mission by providing high quality programs and services within community centers and school partnerships.&nbsp; \\r\\n\\r\\n&nbsp;\\r\\n\\r\\nReporting to the Program Director, the Social Worker is integral to East Side House&rsquo;s strategy to organize resources and share leadership to integrate social services into the fabric of the programs and communities we serve.&nbsp; As a member of Adult and Workforce Development team, Social Workers serve as the primary leaders of wrap-around services for program participants and families, design and implement (proactive and preventive) social work programs to promote social and emotional health, respond to day-to-day participant issues and unanticipated crises and their most immediate needs. &nbsp;This position&rsquo;s direct reports include two case workers and interacts with participants from the Expectant Families and Jobs Plus programs.&nbsp; With a focus on mental health, the Social Worker provides assessments, treatment planning/referrals, crisis intervention and overall coordination of care &ndash; improving mental well-being, strengthening relationships and promoting life skills to assist participants with overcoming barriers preventing goal attainment. &nbsp;&nbsp;\\r\\n\\r\\n&nbsp;\\r\\n\\r\\nEssential Functions and Responsibilities\\r\\n\\r\\nUnder the supervision of the Program Director, the Social Worker, will be responsible for but not limited to the following:\\r\\n\\r\\n\\r\\n\\tComplete initial needs assessment with participant/family.\\r\\n\\tBuild relationships with participating clients through consistent individual/group counseling sessions or mediations.\\r\\n\\tDevelop plan for interventions and will include, but not be limited to, gathering information from program leaders, staff, and members. \\r\\n\\tUse validated tools to screen consenting members for mental health symptoms and re-screen members at appropriate intervals (which will vary, based on population needs and the interventions selected) to identify changes in symptoms and adjust clinical services. \\r\\n\\tFor psychoeducation, facilitate one-on-one sessions and/or group sessions, based on the evidence-based intervention selected. \\r\\n\\tProvide on-site clinical counseling services for participants. On-site services would be delivered in a manner that ensures privacy and confidentiality for those receiving services. \\r\\n\\tProvide community referrals for participants and family as needed. Follow up on referrals to determine outcomes and help resolve barriers.
