    "description_html": "<p>The Children’s Behavioral Health Home (BHH) Coordinator / Targeted Case Manager is responsible for overseeing and coordinating comprehensive care services for children and families with behavioral health needs.</p><p><br></p><p>This <strong>full-time </strong>role blends direct client support with care coordination, documentation, advocacy, and collaboration with multidisciplinary teams to ensure high-quality, person-centered care.</p><p><br></p><p>The ideal candidate is compassionate, organized, and skilled at engaging children and families — including those who may be hard to reach or hard to engage — while navigating complex systems of care.</p><p><br></p><h3><strong>Essential Duties &amp; Responsibilities</strong></h3><h4><strong>Client Assessment &amp; Treatment Planning</strong></h4><ul><li>Establish and maintain therapeutic relationships with clients and families consistent with program philosophy and values<br></li><li>Develop individualized treatment plans in collaboration with families, guardians, and service providers<br></li><li>Monitor and update treatment plans to reflect changing needs<br></li><li>Prepare comprehensive treatment plans, quarterly updates, discharge plans, and required documentation<br></li><li>Support families in identifying goals and coordinating services for their children<br></li></ul><h4><strong>Case Management</strong></h4><ul><li>Provide integrated case management by assessing strengths and needs, coordinating services, and monitoring progress<br></li><li>Lead treatment teams and facilitate quarterly treatment team meetings (or as clinically indicated)<br></li><li>Participate in safety planning and crisis intervention; provide enhanced access during evenings and weekends as needed<br></li><li>Ensure compliance with all regulatory, program, and documentation standards<br></li></ul><h4><strong>Coordination of Services</strong></h4><ul><li>Collaborate with community partners, including mental health providers, hospitals, residential facilities, schools, and social service agencies<br></li><li>Educate families about behavioral health resources and community supports<br></li><li>Ensure timely access to services for all referrals<br></li><li>Advocate for children and families within education, healthcare, and social service systems<br></li></ul><h4><strong>Team Collaboration</strong></h4><ul><li>Attend weekly BHH team meetings and monthly staff meetings<br></li><li>Participate in clinical reviews and professional development opportunities<br></li><li>Maintain productivity expectations and assist with program development initiatives<br></li></ul><h3><strong>General Expectations</strong></h3><ul><li>Maintain confidentiality of client and agency information<br></li><li>Foster a welcoming, inclusive environment for individuals seeking services<br></li><li>Work effectively with individuals from diverse backgrounds and abilities<br></li><li>Demonstrate flexibility to meet client and organizational needs, including occasional overtime<br></li></ul><h3><br></h3><h3><strong>Why Join Our Team?</strong></h3><ul><li>Mission-driven work supporting children and families<br></li><li>Collaborative and supportive team environment<br></li><li>Opportunities for professional growth and development<br></li><li>Commitment to flexibility while maintaining quality care</li></ul>",
