Working as part of a multidisciplinary team, the Rehabilitation Specialist supports clients in achieving stability, independent living, and recovery through a &ldquo;whatever it takes&rdquo; approach consistent with Department of Mental Health (DMH) and Medi-Cal requirements. Services are delivered primarily in the field and include mental health rehabilitation, targeted case management, and crisis intervention&nbsp;</span></p><p><span style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\"><strong><u>Principal Responsibilities:</u></strong></span></p><ol style=\"list-style-type: decimal;margin-left: -0.15000000000000002in;\"><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Conduct field-based work (i.e. County Jail and community based)</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Conduct screenings for all service requests that come in in a timely manner (as defined by DMH Access to Care standards)</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Work collaboratively with a multidisciplinary team to develop a comprehensive care plan and monitor on an ongoing basis. Communicate client progress, needs, and risks to the team</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Provide rehabilitation and skill-building services to support independent living, symptom reduction and community engagement&nbsp;</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Provide group rehabilitation service to clients enrolled in the program</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Provide ongoing case managements services including linkage and referral to both internal programs and external resources</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Accompany clients to appointments (medical, psychiatric, benefits, housing)</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Support clients in accessing public benefits (SSI, General Relief, Medi-Cal)</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Assist clients with housing stabilization and retention</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Provide crisis intervention and de-escalation in community settings</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Maintain productivity requirements to meet contract compliance.</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Complete all charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation on timely basis.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Participate in structured and &ldquo;as-needed&rdquo; case conferences</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Collect necessary data for reporting purposes, as requested</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Participate in agency and community meetings.</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Coordinate outreach activities in and outside of clinic</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Participate in required safety and compliance trainings and apply learned practices in daily work activities.</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.</li><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Perform other duties as assigned</li></ol><p><span style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\"><u><strong>Requirements:</strong></u></span></p><ol style=\"list-style-type: decimal;margin-left: -0.15000000000000002in;\"><li style=\"font-family: tahoma, sans-serif; font-size: 14px; color: rgb(0, 0, 0);\">Bachelor&rsquo;s degree and four (4) years of experience in a mental health setting required.
