Description
Description
Physical Therapists—This Is Why You’re Burned Out.
95% productivity demands. Admin-controlled caseloads. Little room for real clinical judgment. Sound familiar? Burnout in physical therapy is skyrocketing, fueled by unsustainable patient volumes, documentation overload, and administrative pressure.
PACE changes everything. This all-inclusive, community-based model puts you back in control—giving you the freedom to practice with intention, autonomy, and expertise. This is how healthcare should be delivered.
Midland Care Connection
Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
PACE Program
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Physical Therapist
Works as part of an interdisciplinary team to help older adults maintain and improve their mobility, strength, balance, and overall physical function so they can remain safe and independent in their homes and communities. They assess each participant’s physical abilities, develop individualized treatment plans, provide therapeutic exercises and mobility training, monitor progress, adjust care based on needs, and educate participants and caregivers on safe movement, fall prevention, and appropriate home strategies. The PT also collaborates with doctors, nurses, occupational therapists, and other team members to coordinate care and support quality of life for frail elderly participants enrolled in the PACE program.
