This role would be responsible for supporting our Wellness Clinic in Independent Living under the direction of the Wellness nurse practitioner. The ideal candidate would be available to work 2 days a week.</p>\n<p> </p>\n<p><strong>Benefits:</strong></p>\n<ul>\n <li>Wellness Programs</li>\n <li>On-site fitness opportunities</li>\n <li>Employee meal program</li>\n <li>Education assistance<strong><em> </em></strong></li>\n</ul>\n<h2>Responsibilities</h2>\n<ul>\n <li>Performs professional duties in accordance with facility policies/procedures and standards of professional practice, State of New Hampshire Nurse Practice Act, OBRA and HIPAA.</li>\n <li>Collaborates with interdisciplinary team members to provide patient-centered care for independent residents. Participates in interdisciplinary team meetings.</li>\n <li>Assesses and documents changes in residents’ physical or mental status and reports findings to the primary care provider or Wellness Clinic nurse practitioner. Provides complete clinical data to facilitate decision-making.</li>\n <li>Maintains standing visits with residents for routine health monitoring, wound care, and assistance with medication management.</li>\n <li>During resident walk-in clinic hours, independently triages and treats minor illness and injury as appropriate within nursing scope, elevating to a higher level of care when necessary. Responds to urgent resident needs via “R-call” alert system; initiates and supports EMS response as appropriate.  </li>\n <li>Provides leadership and direction for direct caregivers. Supervises direct care through observation and evaluation of direct care results.</li>\n <li>Obtains, administers and documents medications and treatments given to residents following professional guidelines and department policies. Inventories and maintains locked system for narcotics.</li>\n <li>Establishes positive working relationships with team members, residents and family members in order to facilitate open, direct communication.</li>\n <li>Supports, creates, and maintains a positive culture for team members and residents.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Must possess, as a minimum, a Nursing Degree or Diploma from an accredited college, University or nursing school.</li>\n <li>Long term care facility and specialized knowledge of nursing care of the elderly and rehabilitative nursing is preferred.</li>\n <li>Must hold a registered nursing (RN, LPN) license within the State of New Hampshire. Certified in intravenous administration preferred. Must maintain CEU's annually.
