The Community Based Skill Builder works directly with clients in their homes, delivering services using the evidence-based PEARLS model of care. This role is ideal for someone passionate about supporting older adults, community-based work, and collaborative care.</span></p><h3 data-pasted=\"true\"><span style=\"font-size: 16px; font-family: arial, sans-serif;
