Description
Redwood Coast PACE with Humboldt Senior Resource Center is looking for a full-time Licensed Vocational Nurse (LVN) to provide care for our participants. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health.
The schedule for this position is Monday through Friday, 8:30 am to 5:00 pm, with occasional on-call coverage for the triage line.
JOB SUMMARY: The Licensed Vocational Nurse (LVN) provides direct nursing care and care coordination for participants in the PACE program. Working under the direction of the Director of Nursing, the LVN monitors participant health status, administers medications and treatments, performs skilled nursing tasks, and responds to medical needs and emergencies. This role collaborates closely with Primary Care Providers, caregivers, and the interdisciplinary team to ensure comprehensive, participant-centered care. The LVN is also responsible for accurate documentation, regulatory compliance, participant and caregiver education, and contributing to a safe and supportive care environment.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Participant Care and Documentation
Monitors general health status of participants daily and administers treatments in accordance with Primary Care Provider orders.
Communicates with participants’ Primary Care Provider/caregivers regarding participant health concerns and makes recommendations.
Monitors participant nutritional status and refers to the Registered Dietitian when appropriate.
Monitors, administers, and records prescribed medications per Title XXII Regulation 54323 and 78317.
Performs scheduled and as-needed home visits to assess participant health status, provide skilled nursing care, and reinforce care plans.
Assists participants with personal care needs, meals, and other general care duties.
Responsible for managing medical emergencies.
Assists with collecting data on participant assessments, so that they are completed and documented in a timely manner.
Conducts on-going wound care to participants.
Perform blood draws when properly certified.
Administers COVID-19 tests to participants and employees as needed.
Administers vaccines to participants as requested by Primary Care Providers. May also administer vaccines to employees.
Maintains individual participant attendance, skilled nursing care records, medication administration records, and other medical data as necessary to document medical nursing care.
Under the direction of the Director of Nursing, files written and verbal Incident Reports with the CA Department of Aging, the Department of Health Care Services, and Redwood Coast Regional Center upon incident; notifies families or caregivers of incidents.
Provides other nursing services as directed by the Director of Nursing.
Consistently meets or exceeds Redwood Coast PACE targets for productivity.
Consistently meets or exceeds Redwood Coast PACE service targets.
Coordination
Assists the Director of Nursing in updating policies and procedures, as needed.
Coordinates follow up with participants after pharmacy review and reports back to Primary Care Provider, coordinating medication changes as needed.
Provides regular contact with pharmacists and facilitates review of medication delivery system when requested by the Director of Nursing.
In coordination with Home Care Manager, provides training and evaluation of competencies for the Care Partners who are part of the home care team.
Team Support
Monitors safety of the work environment and staff safety practices.
Mentors nursing student interns within LVN scope.
Assists the Director of Redwood Coast PACE and Director of Nursing in facilitating department communication regarding participant, caregiver, employee, and program issues.
Attends weekly staff meetings, participant conference meetings, and in-service trainings.
Demonstrates behaviors consistent with agency values.
Additional duties may be assigned, as needed, that align with the responsibilities of the position and support the vision, mission, and values of the organization.
Participates in the coverage of the on-call triage line, in conjunction with Primary Care Providers.
Provides coverage for the other PACE clinic sites within Humboldt County.
Advocacy and Education
Attends meetings with outside agencies, caregivers, and families. Networks with other agencies on behalf of program and participants.
Reinforces education to participants and families regarding medications, medical conditions, safety, etc. and assists Registered Nurses with providing general health education to community, as requested by the Director of Nursing.
Assists participants in obtaining medical services from other health service providers; schedules transportation as needed.
Qualifications
EDUCATION &/or EXPERIENCE :
Must have a high school diploma or GED Equivalent and successful completion of a Licensed Vocational Nurse Certification program.
Must have at least one (1) year of experience working with a frail or elderly population.
LICENSES OR CERTIFICATES :
Must be a current Licensed Vocational Nurse by the California Board of Vocational Nursing.
Current CPR and First Aid certification within six (6) months of hire.
Valid California Driver’s License, and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition, insured for State minimum liability requirements, and that can be used for the applicable job functions noted above.
PRE-SCREENING REQUIREMENTS:
This position requires the successful completion of a background check.
Pre-employment physical, and PPD skin test (TB screening).
A current Motor Vehicle Report (MVR) is required to determine eligibility for driving on behalf of the organization.
COVID-19 and Influenza (flu) vaccines preferred.

Basic knowledge of computers and web-based applications such as timekeeping, training and email programs, as well as electronic health records.
PHYSICAL REQUIREMENTS:
This position requires the ability to perform a combination of sedentary and physically active duties in both clinical and community-based settings. The LVN must be able to sit, stand, and walk for extended periods; frequently bend, stoop, reach, and twist; and use hands for fine motor tasks such as medication administration, documentation, and operating medical equipment. The role requires the ability to lift, carry, push, and pull up to 50 pounds regularly, and occasionally assist with heavier transfers (with appropriate equipment or assistance), including supporting participants with ambulation, transfers, and wheelchair mobility. The position also requires visual acuity and manual dexterity sufficient to perform clinical tasks, as well as the ability to travel between locations, including driving, and safely navigate a variety of environments such as participant homes and community settings. Also requires the ability to operate a motor vehicle safely and travel to participant homes.
