  "BriefDescription": "The Prevention Navigator will assist the Care Manager and Care Navigator with all aspects of care coordination for SAMHSA HOUSE 2.0 clients: complete initial enrollment (all required SAMHSA and program forms/assessments), promote compliance with appointments and treatment regimens, and encourage self-sufficiency. The Prevention Navigator will assist clients to access services, such as primary care, substance use treatment, mental health counseling, transportation, intensive care management, housing, and vocational/educational resources, through phone calls to clients and providers, and home visits when needed.  The Prevention Navigator will participate in outreach events as needed as well as field-based client outreach.",
