Assesses patient’s mental status through interviews and observations and then develops treatment plans or makes referrals to appropriate services. </p>\n<h2>Responsibilities</h2>\n<p><strong>What You Will Do</strong></p>\n<ul>\n <li>Works with patients and their families to determine goals for treatment.</li>\n <li>Solicits family information, observations and concerns.</li>\n <li>Interviews, observes and develops treatment plans for patients.</li>\n <li>Implements, goals, policies and activities designed to meet program objectives</li>\n <li>Conducts individual and group therapy sessions for patients with mental illness, emotional and substance abuse problems.</li>\n <li>May assess incoming clients and eventually help prepare them for transitioning home.</li>\n <li>Provides referrals to appropriate mental health resources.</li>\n <li>Monitors patient’s treatment progress and keeps track of their mental status.</li>\n <li>Consults with other mental health professionals to provided effective courses of treatment.</li>\n <li>Provides crisis intervention services to patients who are in danger to themselves and others.</li>\n <li>Keeps accurate records of patient records and treatment provided.</li>\n <li>Evaluates and utilizes community mental health resources as needed.</li>\n <li>Keeps abreast of changes in field.</li>\n <li>Performs other duties as requested.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Who You Will Be</strong></p>\n<ul>\n <li>NYSED LMSW or LMHC licensure <strong>OR</strong> other QHP (Qualified Health Professional) status such as MSW-Limited Permit with ability to obtain within 90 days of hire and licensure within one year of hire or MHC-Limited Permit with ability to obtain within 90 days of hire for designated programs/divisions.</li>\n <li>Master’s Degree in Social Work or Mental Health Counseling.</li>\n <li>At least One-Two years demonstrated clinical experience in substance use and/or mental health treatment for <strong>designated programs based on needs of the facility. </strong></li>\n <li>In depth knowledge of NYC diverse network of medical and behavioral health care organizations, including substance use disorders, treatment services, education and employment services, and non-traditional community support groups. </li>\n <li>Computer literacy including proficiency in Microsoft Office Suite and EHR.</li>\n <li>Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.</li>\n <li>Ability to maintain confidentiality.</li>\n</ul>\n<p> </p>\n<p>#li-onsite</p>",
