    "description": "<p><br><br></p>\n<p><span>Since 1985, Albuquerque Health Care for the Homeless has dedicated its mission exclusively to providing services to people experiencing homelessness. AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. Our vision is to live in a world that is just and without homelessness. We believe that homelessness is an issue that can be solved through access to quality health care, adequate and affordable housing, and a living wage.</span></p>\n<p><br></p>\n<p>AHCH currently has an exciting opportunity for a Community Support Worker (CSW) who will work primarily in our new Medical Respite facility at the Gateway Center located at 5400 Gibson SE in Albuquerque. This position will also, on occasion, work at our 1217 1st St location downtown and will be dual certified as a Community Health Worker (CHW) to best meet the needs of the Respite clients.</p>\n<p><br></p>\n<p>Our ideal candidate is someone with strong critical thinking skills, a client-centered mindset, and the ability to work well with diverse populations. Though there are no home visits associated with this CSW position, the candidate must be comfortable with transporting clients in our company vehicles. The caseload will vary dependent on need and can include up to 30 patients.  Candidates must be able to complete documentation of encounters within 72 hours of the visit date.</p>\n<p><br></p>\n<p>AHCH practices a harm reduction approach in all services and we seek candidates that embrace and uphold this philosophy and provide services within a trauma informed framework. We are a housing first, treat first organization that strives to remove as many barriers to health care and housing as possible. </p>\n<p><br></p>\n<p><br></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Duties and Responsibilities:</span></p>\n<ul>\n<li>Assess individual client needs in conjunction with medical team at Medical Respite</li>\n<li>Create individualized care plans in conjunction with a medical provider and with direct input from the client and in accordance with Comprehensive Community Support Services (CCSS) and Medicaid requirements</li>\n<li>Work in an integrated manner with other AHCH programs to optimize client care</li>\n<li>Refer client to appropriate programs and agencies for coordination of wrap-around<br>services in the community</li>\n<li>Develop and deliver discharge plan with medical and/or behavioral health</li>\n<li>Participate in client chart audits for peer review</li>\n<li>Maintain dual certification as Community Support Worker (CSW) and Community Health Worker (CHW)</li>\n<li>Perform duties in a trauma informed and client centered manner</li>\n<li>Participate in planning, reporting, and evaluating program components to meet program guidelines and national standards</li>\n<li>Ensure accurate and appropriate documentation of all client contacts in Electronic Health Record (EHR) within timeframe required by AHCH procedure and billing standards (72 hours from date of service)</li>\n<li>Attend program-specific meetings and training including clinical supervision with Behavioral Health and Primary Care</li>\n<li>Attend organizational meetings and training</li>\n<li>\n</li></ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Minimum Qualifications:</span></p>\n<ul>\n<li>Ability to work in a client centered, trauma informed, and harm reduction manner to assist clients in meeting the goals that they develop</li>\n</ul>\n<p><br></p>\n<ul>\n<li>A bachelor’s degree in human service field from an accredited university and one-year relevant experience with vulnerable populations</li>\n<li><span style=\"text-decoration: underline\">OR</span></li>\n<li>An associate’s degree and a minimum of two years of experience working with vulnerable populations</li>\n<li><span style=\"text-decoration: underline\">OR</span></li>\n<li>A high school graduation or general educational development (GED) test and a minimum of three years of experience working with vulnerable populations</li>\n</ul>\n<p><br></p>\n<ul>\n<li>Valid NM Driver's license and clean driving record</li>\n<li>Attain CCSS certification within 90 days of start date</li>\n<li>Attain CHW certification within 90 days of start date</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Preferred Qualifications:</span></p>\n<ul>\n<li>Current CCSS certification</li>\n<li>Current CHW certification</li>\n<li>Experience in trauma-informed care and harm reduction models</li>\n<li>Bilingual in a regionally relevant language<br>\n<ul>\n<li>\n</li></ul>\n<br>\n<ul></ul>\n<span style=\"font-weight: bold\">All benefits start the month after you begin work:</span></li>\n</ul>\n<p><br></p>\n<ul>\n<li><span>Low cost medical, vision, and dental insurance with health club membership</span></li>\n<li><span>Life insurance and Accidental Death and Dismemberment fully paid for by organization</span></li>\n<li><span>Long Term Disability fully paid for by organization</span></li>\n<li><span>Paid Time Off - 24 days in first year of employment</span></li>\n<li><span>Catastrophic Sick Time accrual</span></li>\n<li><span>7 Paid holidays</span></li>\n<li><span>Health Care and Dependent Care Flexible Spending Accounts</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">401k with employer match</span><br></li>\n<li><span>Student loan forgiveness eligible </span></li>\n</ul>\n<p><br><br></p>\n<p><span><em>We're an equal opportunity employer.
