    "ExternalDescriptionStr": "<p><span>Primary Accountability</span></p>\n<p><span>The Internist is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care.</span></p>\n<p><br><span><strong>Description of Primary Responsibilities:</strong></span></p>\n<ol>\n <li><span>Responsible for the direct provision of primary care medical services meeting all established productivity expectations.</span>\n  <ul>\n   <li><span>Will provide examination and will review appropriate laboratory, referral and imaging results to determine a diagnosis.</span></li>\n   <li><span>Expected to document visits accurately utilizing appropriate templates, and record relevant data to attain a diagnosis.</span></li>\n   <li><span>Accountable for attaining budgeted visits.</span></li>\n   <li><span>Accountable for meeting clinical care measures.</span></li>\n   <li><span>Accountable for locking records in the 72 hour window.&nbsp;</span></li>\n  </ul></li>\n <li><span>Responsible for Internal Medicine professional focus and goals:</span>\n  <ul>\n   <li><span>Provide comprehensive, continuous, and coordinated care managing both common and complex illness adults, and the elderly;</span></li>\n   <li><span>Comprehensive health care including health promotion and maintenance, prevention of illness and disability, and application of a holistic healthcare model which addresses both protective factors and risk factors affecting the health and wellbeing of patients;</span></li>\n   <li><span>Chronic disease management skills are utilized to provide a team-based, patient-centered approach to the planned care of chronic conditions with the goals of maximizing quality of life and health care; where the internist acts as a diagnostician, a therapist, a counselor, and a coordinator of care; and</span></li>\n   <li><span>Understanding in the diagnosis and treatment of cancer, infections, and diseases affecting the internal organs with incorporation of essentials of primary care internal medicine with focus of disease prevention, wellness, substance abuse, mental health, and effective approach to both health beliefs and comprehensive care needs of patients.</span></li>\n  </ul></li>\n <li><span>Accountable for providing excellent customer service and care.&nbsp;</span>\n  <ul>\n   <li><span>Consistently behaves with courtesy when interacting with patients/ family members, support staff, and Network staff.</span></li>\n   <li><span>Provider remains aware of wait time during the daily workflow.</span></li>\n   <li><span>Responsible for providing the patient with educational materials and resources appropriate to the patient’s health literacy needs.</span></li>\n  </ul></li>\n <li><span>Accountable for patient status as measured by relevant clinical quality measures.</span>\n  <ul>\n   <li><span>Promotes and participates in daily huddles.&nbsp;</span></li>\n   <li><span>Incorporates the participation of ancillary health team members in managing the care of patients / families.</span></li>\n   <li><span>Responsible for setting and monitoring self- management plans.</span></li>\n   <li><span>Responsible for assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.</span></li>\n   <li><span>Completes chart and peer review responsibility in a timely manner.</span></li>\n  </ul></li>\n <li><span>Assists the patient care team with performance improvement efforts, staff education, and FHCN promotion.</span>\n  <ul>\n   <li><span>Responsible for assisting the patient care team with performance evaluations and quality improvement.</span></li>\n   <li><span>Responsible for assisting in the preceptorship of mid-level practitioners.</span></li>\n   <li><span>Where relevant participate in rotation of medical, physician assistant students, and dental residents.</span></li>\n   <li><span>Participates in Network Health Fairs &amp; other community events.&nbsp;</span></li>\n   <li><span>Participates in site &amp; provider meetings in a meaningful way.</span></li>\n  </ul></li>\n <li><span>Meets Network and credentialing expectations for licensure, and employment.</span></li>\n <li><span>Performs other duties as assigned.</span></li>\n</ol>\n<p><span><strong>Description of Primary Attributes</strong></span></p>\n<p><span>Professional &amp; Technical Knowledge:</span></p>\n<ol>\n <li><span>Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training in Internal Medicine, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a Doctor of Medicine or Doctor of Osteopathic Medicine degree.</span></li>\n</ol>\n<p><span>Technical Skills:</span></p>\n<ol>\n <li><span>Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.&nbsp;</span></li>\n <li><span>Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.</span></li>\n <li><span>Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.</span></li>\n</ol>\n<p><span>Licenses &amp; Certifications: Licensed in the state of California as an Internist. Board-eligibility or certification in Internal Medicine may be required or desired in certain positions. &nbsp;&nbsp;</span></p>\n<p><br><span>Communications Skills:</span></p>\n<ol>\n <li><span>Requires the employee to effectively communicate their professional opinions and extrapolations of information they collect and synthesize/analyze.</span></li>\n <li><span>Employees must determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.&nbsp;</span></li>\n <li><span>Duties require the preparation and execution of presentations to large groups.</span></li>\n</ol>\n<p><span>Physical Demands: The physical demands described here in this job description are representative of those that must be met by an employee to successfully perform the essential functions of this position.
