    "requisitionDescription": "<div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><p data-pasted=\"true\"><strong>Location:</strong> New Philadelphia and Wooster Service Areas<br><strong>Position Type:</strong> PRN (As Needed)<br><strong>Schedule:</strong> Evening and Weekend Coverage</p><h3>Position Overview</h3><p>Ohio&rsquo;s Hospice is seeking a compassionate and dedicated LPN or RN to provide after-hours nursing support for patients receiving hospice care in the New Philadelphia and Wooster communities. This driving position offers an opportunity to make a meaningful difference in the lives of patients and families by delivering exceptional care during some of their most important moments.</p><p>The ideal candidate is committed to hospice philosophy, values patient-centered care, and is comfortable working independently while collaborating with an interdisciplinary care team.</p><h3>Schedule Expectations</h3><ul data-spread=\"false\"><li>Evening shifts: 4:00 PM &ndash; 12:00 AM, one to three shifts per week</li><li>Weekend coverage: 8:00 PM &ndash; 8:00 AM every other weekend</li><li>Participation in a rotating holiday schedule is required</li><li>Travel throughout the assigned service area is expected</li></ul><h3>Key Responsibilities</h3><ul data-spread=\"false\"><li>Deliver skilled nursing care to hospice patients in homes, assisted living communities, and other care settings</li><li>Assess patient needs and provide interventions that support comfort, dignity, and quality of life</li><li>Collaborate with Registered Nurses and other members of the hospice care team to implement individualized care plans</li><li>Educate and support patients, caregivers, and family members regarding care needs and symptom management</li><li>Maintain timely and accurate electronic documentation</li><li>Respond professionally and compassionately to changing patient conditions</li></ul><h3>Qualifications</h3><ul data-spread=\"false\"><li>Current Ohio LPN or RN license in good standing</li><li>Minimum of one year of nursing experience preferred</li><li>Current CPR certification</li><li>Proficiency with electronic documentation and basic computer applications</li><li>Ability to travel independently and safely during daytime, evening, overnight, and adverse weather conditions</li><li>Valid driver&#39;s license with an acceptable driving record</li><li>Proof of state-required automobile insurance coverage</li><li>Successful completion of a criminal background check</li><li>Successful completion of a 10-panel drug screening</li></ul><h3>Why Join Ohio&rsquo;s Hospice?</h3><p>At Ohio&rsquo;s Hospice, our mission is centered on providing superior care and superior services to every patient and family we serve.
