General Summary: The Case Management Assistant assists the Case Management Nurses with scheduling patient appointments for hospital follow-ups and for the High-Risk Team. The Case Management Assistant will act as a float and on occasion will provide direct and indirect patient care activities in the patient's home alongside the Physician Assistant .
Supervisory Responsibilities: This position has no supervisory responsibilities.
General Requirements: All duties performed will be done accurately and in a timely manner.
Based on hospital discharges; contacts patients / families to get them scheduled for timely hospital follow-up appointments.
Communicates patient concerns promptly to the case management nurses for attention and follow-up.
Schedules patients to the High-Risk Team of providers for appropriate visit / care based on HRT audit.
Assists in tracking high risk patients and completion of audit plan.
Receives inbound calls to assist patients with health plans or care related.
Assists patients with information about community resources available to support their healthcare / social need.
Arrange transportation for patients to / from health care visits.
Maintains strictest member and company confidentiality.
Assumes responsibility for maintaining clinical competencies according to Gonzaba Medical Group Policy.
Cleans, stocks, and maintains instruments and equipment.
Essential Job Responsibilities:
This position will require driving a company vehicle or a personal vehicle; therefore, employees must successfully complete a motor vehicle history check, possess, and maintain a current valid Texas Driver License, and proof of current insurance to be subject for mileage reimbursement.
Performs patient care in the home setting. Functions within the scope of training received from Medical Assistant Education program and from Gonzaba Medical group programs such as monitoring vital signs, applying monitoring devices such as pulse oximeter, venipuncture, glucose monitoring, etc. Strong phlebotomy skills are essential.
Reports and accurately documents clinical findings to provider and/or Physician.
Transports patients safely from the following positions: sitting to standing or laying down, from standing to sitting or lying down, from laying down to standing or sitting, and assist to the bathroom, into/out of wheelchairs, and pushing/pulling patients in wheelchairs.
Must always keep the patients’ safety a priority, which will involve preventing potential falls as needed.
Education and Training: Minimum high school education or equivalent. Certificate of completion from an accredited school for medical assistants is required or equivalent medical training. CPR certification with AED is required and must be kept current. National certification as a Registered or Certified Medical Assistant is required as initial and continued employment at Gonzaba Medical Group is contingent upon this credential.
National Certification must be issued from one of the following associations: AAMA - Certified Medical Assistant (CMA); ARMA - Registered Medical Assistant (RMA); NAHP - Nationally Registered Certified Medical Assistant (NRCMA); or other association approved by GMG Human Resources.
Experience: Experience in a clinic, doctor’s office or hospital with a minimum of one-year experience preferred.
Other Requirements: Computer Skills : Skilled in use of computer/EMR systems. Knowledge of Word processing software, spreadsheet software, Internet, and database software.
Work Environment: Depending upon the area assigned may be 100% clinical setting or office setting in a clinic environment. Exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.
Mental / Physical Requirements: Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting up to 40 lbs. without assistance. Close vision and ability to adjust focus. Must be able to work efficiently under pressure.
