Grow with us.</span></p>\n<p><span style=\"font-weight: bold\">Licensed Practical Nurse <br>                                    Name: <br>                                    Hourly, non-exempt<br>                                    1.0 FTE<br>                                    Reports to: Director of Nursing</span><br></p>\n<ul>\n<li>Provide patient care to patients across settings in private home, adult foster home, residential/assisted living care, and/or nursing facility following existing care plans.</li>\n<li>Contribute to patient assessment under the supervision of a registered nurse by assisting in identifying changes in patient status.</li>\n<li>Provides nursing care as needed; attentive to physical, emotional, and spiritual needs of patient.</li>\n<li>Counsel patient and family regarding symptom control, medications, direct nursing care measures, nutrition, etc.</li>\n<li>Assists patient and family in making choices that best meet their needs by providing alternatives and support.</li>\n<li>Attends deaths, supports families, assists with final arrangements.</li>\n<li>Obtain necessary physician orders and collaborate with RN and attending physician on patient needs and follow up on and obtain lab results and notify attending physician of results.</li>\n<li>Communicates often and well with other Lumina staff, functions as an integral member of the patient care team.</li>\n<li>Provides all services in full compliance with Medicare regulations, including timely, professional documentation.</li>\n<li>Contributes to communication, positive work environment, and team effort.</li>\n<li>Serves as assigned on agency-wide initiatives and/or committees.</li>\n<li>Understands and adheres to Lumina Privacy Policy; Lumina Vulnerable Persons Policy; and all policies and procedures contained in the Lumina Employee Handbook.</li>\n<li>Monitor signs and symptoms as well as reactions to interventions, treatments, and medications under the direction of the RN.</li>\n<li>Utilize appropriate standards of care for the identified medical diagnosis.</li>\n<li>Utilize Universal Precautions and safety measures to ensure a safe care environment for patients including consistent use of personal protective equipment (PPE).</li>\n<li>Demonstrate competencies and knowledge related in the standards of practice for a licensed practical nurse in the state of Oregon when providing care.</li>\n<li>Utilize and become proficient in navigating the electronic medical records system for documenting and reviewing patient conditions and plan of care.</li>\n<li>Demonstrate an understanding and respect for patient rights when providing care.</li>\n<li>Establish and maintain effective working relationships with the patients and families, co-workers, and members of diverse cultural backgrounds.  Know and apply principles of an inclusive work environment.</li>\n<li>Complete annual mandatory education review and competency programs.</li>\n<li>Other duties as assigned.</li>\n</ul>\n<p> </p>\n<p><span style=\"font-weight: bold\">Working conditions and special requirements: </span></p>\n<ul>\n<li>Routine hours  within the 8-5, Monday through Friday work week. Participates in on call rotation as needs for organization coverage of after hour services and holidays  </li>\n<li>Provides care in a variety of settings, including homes with unpredictable environments, including uneven surfaces and potential hazards such as animals, smoke, and exposure to communicable diseases etc.</li>\n<li>Full access to an insured vehicle for work with expectation of significant driving, including at night.</li>\n<li>Must be able to safely provide care for patients of many sizes and functional abilities. Must be able to lift at least 50 lbs without assistance and able to roll by pushing or pulling a 200 lbs. or more person who can assist or 150 lbs. person who cannot assist. Able to physically provide care to dependent patients who may be frail, elderly, obese, and or physically impaired; provide up to total assist with patient positioning, transfers and ambulation.</li>\n<li>Physical, intellectual, and emotional capability to perform all essential functions of position.
