In this role you will be responsible for conducting assessments, developing and delivering treatment plans focused on achieving the goals of care including mobility and function, symptom management, and delivering education.</p><ul><li>Work under the supervision of the Program Director</li><li>Collaborate with the interdisciplinary team to create personalized care plans based on the patient’s unique needs and goals of care</li><li>Participate in clinical huddles and case conferences with team members such as physicians, nurses, social workers, speech therapists and occupational therapists</li><li>Conduct physiotherapy assessment, consultation and planned interventions for patients</li><li>Engage in skilled, compassionate communication with patients, families, and caregivers regarding mobility, and function</li><li>Educate and mentors patients, family and caregivers on strategies to maintain safe mobility and function in the home setting</li><li>Maintain accurate and timely documentation of patient assessments, treatment plans, and outcomes, ensuring adherence to ethical and legal standards</li></ul>",
