This role is responsible for conducting functional assessments, developing treatment plans, leading therapeutic interventions, and facilitating discharge planning to ensure continuity of care. The Behavioral Health Therapist plays a key role in patient and family engagement, crisis intervention, and care coordination across the continuum.<br><br><strong>Essential Functions</strong></p><ul><li>Completes timely and accurate psychosocial assessments and functional evaluations for assigned patients.</li><li>Develops individualized plans of care based on patient needs, and actively participates in multidisciplinary treatment team meetings.</li><li>Communicates effectively with patients, families, medical providers, referral sources, and community partners to support coordination and continuity of care.</li><li>Facilitates therapeutic interventions including individual, group, and/or family therapy, based on scope of practice and patient needs.</li><li>Supports discharge planning and resource navigation for post-hospital care.</li><li>Documents all clinical interventions and communications in accordance with facility, state, and federal regulations.</li><li>Participates in staff development, orientation, and education of assigned personnel or students.</li><li>Responds appropriately to crisis situations, including implementation of de-escalation strategies and emergency response protocols.</li><li>Reports and documents unexpected incidents or adverse events, supporting investigations and corrective actions when needed.</li><li>Performs other duties as assigned.</li><li>Maintains regular and reliable attendance.</li><li>Complies with all policies and standards.</li></ul><p><strong>Qualifications</strong></p><ul><li>Master's Degree in Social Work, Counseling, Psychology, or related behavioral health field required</li><li>1-3 years prior behavioral health experience preferred</li></ul><p><strong>Knowledge, Skills and Abilities</strong></p><ul><li>Knowledge of human behavior, symptoms of mental illness, and therapeutic interventions.</li><li>Proficient in de-escalation, crisis management, and stress reduction techniques.</li><li>Strong understanding of managed care and community resource coordination.</li><li>Ability to collaborate effectively with interdisciplinary teams.</li><li>Skilled in documentation, communication, and patient education.</li></ul><p><strong>Licenses and Certifications</strong></p><ul><li>LMSW - Licensed Medical Social Worker - State Licensure Preferred or</li><li>LPC-Licensed Professional Counselor Preferred</li><li>BCLS - Basic Life Support required</li><li>De-escalation training certification within 30 days of hire required</li><li>ACLS - Advanced Cardiac Life Support preferred</li></ul><p>INDBH</p>",
