    "description": "<h2>Overview</h2> Good Shepherd Hospice is seeking a compassionate \n<strong>Hospice Intake Coordinator</strong> to serve in our \n<strong>Olathe</strong>\n<strong> and surrounding communities.</strong>The Hospice Intake Coordinator assists in gathering required patient information and coordinating enrollment of new patients into hospice program.\n<ul>\n <li><strong>Monday-Friday, 8:00am-5:00pm</strong></li>\n <li><strong>Hospice Admissions/Intake experience required</strong></li>\n <li><strong>Homecare Homebase software experience highly preferred</strong></li>\n</ul>\n<h2>Responsibilities</h2>\n<ul>\n <li>Initial contact person for all new inquiries and referrals</li>\n <li>Responds promptly to any referral or inquiry</li>\n <li>Coordinates the scheduling of intake personnel to follow up with referrals</li>\n <li>Provides guidance to hospice staff during the referral and enrollment process</li>\n <li>Initiates communication with the RN Case Manager regarding eligibility and enrollment into hospice program</li>\n <li>Informs the RN Case Manager of unusual or potentially problematic patient/family issues</li>\n <li>Coordinates all admission enrollment paper work</li>\n <li>Obtains history and physicals, progress notes, lab, nurse’s notes, medication sheets, etc. as needed to complete initial patient assessment</li>\n <li>Maintains regular communication with the referral source</li>\n <li>Reviews completed enrollment packet for accuracy and forwards for billing and filing</li>\n <li>Inputs initial referral information into the computer</li>\n <li>Communicates with medical professionals, the hospice team and others</li>\n <li>Coordinates completion of admission packets</li>\n <li>Maintains accurate, up-to-date records</li>\n <li>Demonstrates strong communication and customer service skills</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Hospice Intake experience required</li>\n <li>Valid driver’s license</li>\n <li>Demonstrated commitment to the hospice philosophy of care</li>\n <li><strong>Can successfully pass background checks & drug screen</strong></li>\n</ul>\n<p>#IND1</p>",
