It is an initiative aimed at ensuring that veterans have access to timely and high-quality healthcare services, even if the VA facilities are unable to provide the necessary care or if it is more convenient for the veteran to receive care in their local community. VA provides care to Veterans through community providers when VA cannot provide the care needed. Community care is based on specific eligibility requirements, availability of VA care, and the needs and circumstances of individual Veterans. The community care consultants work with a variety of disciplines but one of their main focus is the Group Practice Manager (GPM), RCT coordinators, Community Care Nurses and other Referral Management focused roles. Community care is a majority Ambualtory focused solution. It is heavily involved in ambulatory build with a focus on Community Care specific Mpages and documentation. Referral Management is another area of build that the team maintains and builds relating to Worklists, practice sites and other associated build.</p>",
