We are seeking a Music Therapist for our Memory Care and Transitional Care Unit at our Skilled Nursing Facility. We have a great Therapeutic Recreation Team and as we grow our community with new residents, we want to provide holistic engagement opportunities for them.</p>\n<p> </p>\n<p>At Catholic Eldercare, we offer a range of benefits including educational scholarship opportunities, health, dental, vision, life, short-term disability and long-term disability insurance, flexible spending account, retirement savings and great work/life balance. </p>\n<p>EEO/AA</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Provides individualized music therapy services to meet physical, emotional, cognitive, psychosocial and spiritual needs of residents living on memory care floor. </li>\n <li>Creates, develops and leads a variety of large and small groups based on resident interests.</li>\n <li>Works with local University music therapy programs to supervise practicum students as part of professional development </li>\n <li>Participates in the planning and facilitation of monthly activities calendars on Memory care unit in collaboration with activity coordinators. </li>\n <li>Assists TR department with parties other in-house events. </li>\n <li>Maintains accurate documentation of services provided and other necessary information in resident charts</li>\n <li>Good communication skills and the ability to multi-task and adapt to work environment as needed.</li>\n <li>Compassion and caring attitude needed for elders that may have a variety of emotional, physical, cultural and dementia related needs. </li>\n <li>Provides updates to Memory Care TR coordinator for weekly Care Conferences as part of the interdisciplinary health care team. </li>\n <li>Completes MDS Assessments, Care Plans, and Progress notes for TCU guests</li>\n <li>Plans and facilitates 1:1s and small groups for TCU guests to meet their needs during their stay. </li>\n <li>Determines supplies and equipment needs and notifies Director or purchases per Director approval.</li>\n <li>Maintains order and cleanliness of activity room and storage areas.</li>\n <li>Follows all company policies and procedures and complies with all required training; complies with all state, federal laws, rules and regulations. </li>\n <li>Must be able to push a wheelchair, walk, sit and stand throughout the day. </li>\n <li>Ability to work with staff, family, volunteers and vendors in a professional manner.</li>\n <li>Work as a team member and adapt to changes in work assignments to meet resident and/or staffing needs.</li>\n <li>Performs other related duties as assigned.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Graduate of an accredited music therapy program with Bachelor’s degree in Music Therapy or its equivalence.  </li>\n <li>Completion and passing (or eligibility) of National Certification Exam (CBMT) credentials of MT-BC Music Therapy Board Certified required.</li>\n <li>Experience in a long-term care environment with adults 62+, preferred</li>\n <li>Proficiency in Guitar, Piano & voice</li>\n</ul>",
