We provide Medication-Assisted Treatment (MAT) and behavioral health services, helping patients discover their purpose and build a healthy community. We are seeking a dedicated and compassionate </span><span>Patient Care Coordinator (PCC)</span><span> to join our growing team in a fast-paced, dynamic medical setting. This entry-level position offers comprehensive training and the opportunity to make a direct impact on the lives of patients.</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Responsibilities and Duties:</span><span><br>As a Patient Care Coordinator, you will be trained to support the professional medical staff and help manage clinic operations. Key responsibilities include:</span></p>\n<ul>\n<li><span>Patient Scheduling:</span><span> Coordinate patient appointments and ensure timely and accurate scheduling.</span></li>\n<li><span>Collecting Vitals &amp; Patient Information:</span><span> Take and record vital signs (e.g., blood pressure, temperature, heart rate) and gather essential patient information.</span></li>\n<li><span>Phone Handling &amp; Patient Interaction:</span><span> Answer phone calls, assist patients with inquiries, and provide direction as needed.</span></li>\n<li><span>Patient Registration:</span><span> Admit and register patients into the clinic system, ensuring all necessary documentation is completed.</span></li>\n<li><span>Data &amp; Record Keeping:</span><span> Accurately input patient data into the clinic’s electronic health record (EHR) system and maintain updated patient files.</span></li>\n<li><span>Facility Maintenance:</span><span> Assist with building upkeep, cleaning, and ensuring the clinic environment is safe and welcoming.</span></li>\n<li><span>Communication with Medical Staff:</span><span> Relay information between patients and medical providers, ensuring smooth communication and efficient care.</span></li>\n<li><span>Customer Service &amp; Patient-Centered Care:</span><span> Provide exceptional service to patients by assisting with inquiries, directing them to appropriate services, and fostering a compassionate and supportive clinic atmosphere.</span></li>\n<li><span>Crisis Management &amp; De-escalation:</span><span> Demonstrate competency in verbal de-escalation, crisis intervention techniques, and maintaining a calm, professional demeanor in stressful situations.</span></li>\n<li><span>Regulatory Compliance:</span><span> Follow HIPAA, OSHA, and other safety protocols to ensure patient confidentiality and a safe work environment.</span></li>\n</ul>\n<p><span>Additionally, employees will be expected to develop a high-level understanding of:</span></p>\n<ul>\n<li><span>Medication-Assisted Treatment (MAT)</span></li>\n<li><span>Harm Reduction and Low Barrier/High Access Treatment</span></li>\n<li><span>Patient confidentiality (HIPAA)</span></li>\n<li><span>Safety practices (OSHA)</span></li>\n<li><span>Crisis Intervention and De-escalation</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Qualifications and Skills:</span></p>\n<ul>\n<li><span>Great entry level position for anyone seeking a start in the medical field</span></li>\n<li><span>Previous Experience:</span><span> Experience in a medical clinic or addiction treatment setting is preferred. However, willingness to learn and grow in the field is equally valued.</span></li>\n<li><span>Education:</span><span> High school diploma or GED equivalent required. (Rovering is on a week on, week off basis (alternating) and is as follows:<span style=\"color: rgb(29, 28, 29)\"> Tuesdays- Granby, Kremmling,  Wednesday-Steamboat, Thursday- Hayden, Meeker.
