Summary</span></span></p>\n<p><span style=\"font-size: 16px;\">To provide one-on-one in-home rehabilitative services to adult clients with mental illness.<span>  </span>Rehabilitative services enable clients to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, independent living, and community skills, and provide resources and referrals.<span>  </span></span></p>\n<p><span style=\"font-size: 16px;\"> </span></p>\n<p><span style=\"font-weight: bold;\"><span style=\"font-size: 16px;\">Duties and Responsibilities</span></span></p>\n<p><span style=\"font-size: 16px;\"> </span></p>\n<ul>\n<li><span> </span><span>Develop, implement, evaluate, and revise treatment goals </span></li>\n<li><span> </span><span>Provide ongoing services to individuals within their homes or community </span></li>\n<li><span> </span><span>Must be able to travel within the Central Minnesota Region</span></li>\n<li><span> </span><span>Encourage client relationship and regular communication, coordinate services with other health care providers, interpreter, vocational workers, etc to ensure that client receives quality service to meet their needs</span></li>\n<li><span> </span><span>Maintain a caseload and be able to sustain attention to detail</span></li>\n<li><span> </span><span>Ability to work independently, be flexible and able to meet deadlines</span></li>\n<li><span> </span><span>Accurately document services and complete progress notes and submit within 24-48hours</span></li>\n<li><span> </span><span>Complete Functional Assessments, Independent Treatment Plans, and/or other required forms and documents in a timely manner</span></li>\n<li><span> </span><span>Attend group/individual clinical supervision meetings/trainings</span></li>\n<li><span> </span><span>Meet with supportive supervision as needed</span></li>\n<li><span> </span><span>Ensure compliance with DHS regulations</span></li>\n<li><span> </span><span>Communicate effectively and respectfully with supervisor, office staff, clients, vendors, and clients.</span></li>\n<li><span> </span><span>Mandated reporter</span></li>\n<li><span> </span><span>Provide quality services for clients, and assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans with the collaboration of the client and the treatment team</span></li>\n<li><span> </span><span>Proactively and efficiently communicating with clients, interpreters, and other parties if sessions are canceled or rescheduled</span></li>\n<li><span> </span><span>Must complete 15 hours per year of ongoing continuing education in areas of mental illness and mental health services</span></li>\n<li><span> </span><span>Communicate effectively and in a timely manner with Credent Care staff, other practitioners, vendors, clients, interpreters, and other agency workers. </span></li>\n</ul>\n<p><span style=\"font-size: 16px;\"> </span></p>\n<p><span style=\"font-weight: bold;\"><span>Scope of the Position</span></span></p>\n<p><span>A mental health professional has full authority to independently practice, within the scope of practice of their professional license, and accordingly to Company policies and procedures and relevant ARMHS rules and regulations.<br><br></span></p>\n<ul>\n<li><span>A mental health professional must maintain a valid license with the mental health professional's governing health-related licensing board and must only provide services to a client within the scope of practice determined by the applicable health-related licensing board.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold;\"><span>Qualifications</span></span></p>\n<p><span>Must meet DHS qualifications for Mental Health Practitioner in one of the following ways:</span></p>\n<ul>\n<li><span> </span><span>Has completed at least 30 semester hours or 45 quarter hours in behavioral sciences</span></li>\n</ul>\n<p><span>or related fields and has at least 2,000 hours of supervised experience in the delivery</span></p>\n<p><span>of services to adults or children with:</span></p>\n<ul>\n<li><span> </span><span>Mental illness, substance use disorder, emotional disturbance, or</span></li>\n<li><span> </span><span>Traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness, and substance abuse, and psychotropic medications and side effects.</span></li>\n</ul>\n<ul>\n<li><span> </span><span>Has at least 4,000 hours of supervised work experience in the delivery of services to adults or children with any of the following:</span>\n<ul>\n<li><span> </span><span>Mental illness, substance use disorder, emotional disturbance, or</span></li>\n<li><span> </span><span>Traumatic brain injury or developmental disabilities and completes training on mental</span></li>\n<li><span> </span><span>illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness, substance abuse, and psychotropic medications and side effects.</span></li>\n</ul>\n</li>\n<li><span> </span><span>Has at least 2,000 hours of supervised experience in the delivery of services to adults</span></li>\n</ul>\n<p><span>or children with any of the following:</span></p>\n<ul>\n<li><span style=\"font-size: 13px;\"> </span><span>Mental illness, substance use disorder, emotional disturbance, or</span></li>\n<li><span style=\"font-size: 13px;\"> </span><span>Traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness, substance abuse, and psychotropic medications and side effects; and receives clinical supervision as required at least once a week from a mental health professional until the requirement of 4,000 hours of supervised experience is met.</span></li>\n<li><span> </span><span>Has a graduate student internship or a bachelor’s or master’s degree and is a graduate student in behavioral sciences or related fields and is formally assigned by an accredited college or university to an agency or facility for clinical training</span></li>\n<li><span> </span><span>Has a bachelor’s or master’s degree with any of the following:</span></li>\n<li><span style=\"font-size: 13px;\"> </span><span>Holds a master’s or other graduate degree in behavioral sciences or related fields</span></li>\n<li><span style=\"font-size: 13px;\"> </span><span>Holds a bachelor’s degree in behavioral sciences or related fields and completes </span></li>\n<li><span style=\"font-size: 13px;\"> </span><span>practicum or internship that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields.</span></li>\n</ul>\n<ul>\n<li><span>A Mental Health Practitioner for an adult member must have training working with adults.</span></li>\n<li><span>Knowledge of the effects of mental illness on people with cognitive impairments.</span></li>\n<li><span>Knowledge of community resources.</span></li>\n<li><span>Must have the ability to work independently, enjoy flexibility, and manage stress.</span></li>\n<li><span>Proficient in the use of Procentive to document clinical care.</span></li>\n</ul>\n<p><span> Skills &amp; Abilities</span></p>\n<ul>\n<li><span>Be able to
