    "requisitionDescription": "<div><div><div><p id=\"isPasted\"><strong>Position:</strong> Licensed Practical Nurse (LPN)&nbsp;<strong>Department:</strong> Home Health<br><strong>Supervisor:</strong> Home Health Clinical Team Lead</p><p><strong>Job Summary</strong></p><p>The Licensed Practical Nurse (LPN) provides skilled nursing care to Home Health patients, families, and caregivers under the direction of a Registered Nurse. This position ensures that patient care is delivered in accordance with the plan of care, Medicare Conditions of Participation, Pennsylvania licensure regulations, and agency policies. The LPN plays a critical role in supporting patients in the home environment, promoting comfort, healing, and independence.</p><p><strong>Primary Responsibilities</strong></p><p><strong>Clinical Care</strong></p><ul type=\"disc\"><li>Deliver safe, high-quality nursing care as outlined in the patient&rsquo;s plan of care.</li><li>Administer medications and treatments in accordance with the PA Nurse Practice Act.</li><li>Monitor and report patient responses to medications and treatments.</li><li>Assess and educate patients and caregivers on disease management, medications, and self-care techniques.</li><li>Assist patients with activities of daily living, promoting independence and comfort.</li><li>Observe for changes in condition and report to RN Case Manager or Clinical Manager.</li><li>Maintain timely and accurate documentation of all care provided.</li><li>Participate in coordination of care with the interdisciplinary team to ensure continuity.</li><li>Provide visits to Hospice patients as needed.</li></ul><p><strong>Team Collaboration &amp; Communication</strong></p><ul type=\"disc\"><li>Communicate effectively and respectfully with patients, caregivers, and all members of the care team.</li><li>Serve as a mentor during orientation or field training as requested.</li><li>Promote a supportive and respectful work environment aligned with agency values.</li><li>Attend staff meetings, case conferences, and required in-services.</li></ul><p><strong>Regulatory &amp; Professional Standards</strong></p><ul type=\"disc\"><li>Maintain compliance with all local, state, and federal regulations.</li><li>Comply with Medicare Home Health Conditions of Participation and agency procedures.</li><li>Maintain confidentiality in all patient and agency-related matters.</li><li>Uphold professional and ethical conduct in all responsibilities.</li><li>Participate in agency quality improvement initiatives and documentation reviews.</li></ul><p><strong>&nbsp;</strong></p><p><strong>Resource Management</strong></p><ul type=\"disc\"><li>Coordinate visits efficiently and manage caseloads cost-effectively.</li><li>Maintain clinical records according to agency standards.</li><li>Participate in weekend/evening rotation as required.</li></ul><p><strong>Qualifications</strong></p><ul type=\"disc\"><li>Graduate of an accredited practical nursing program</li><li>Licensed Practical Nurse (LPN) licensed in Pennsylvania</li><li>One year of clinical experience preferred (Home Health, Med/Surg, or critical care)</li><li>CPR certification required</li><li>Valid PA driver&rsquo;s license and reliable, insured transportation</li><li>Satisfactory background check and references</li></ul><p><strong>Physical Requirements and Working Conditions</strong></p><ul type=\"disc\"><li>Ability to travel between patient homes and facilities</li><li>Manual dexterity for documentation and telephone use</li><li>Ability to lift up to 50 pounds occasionally</li><li>Adequate visual/hearing ability for patient care and communication</li><li>Ability to work in varied home and facility environments</li></ul><p><strong>Core Values</strong></p><p><strong>Integrity</strong> &ndash; We build trust through responsible actions and honest relationships.<br><strong>Respect</strong> &ndash; We value the potential in each individual and treat all with dignity and compassion.<br><strong>Teamwork</strong> &ndash; We work together without barriers and encourage creativity through empowerment.<br><strong>Appreciation</strong> &ndash; We foster a culture that recognizes and rewards employee contributions.</p></div></div></div>\n",
