Assesses the patients' eligibility for home care, explaining services and benefits, completing admission paperwork, and ensuring a smooth transition for home care services. Works closely with the interdisciplinary teams to provide comprehensive and compassionate care.</p>\n<h2>Qualifications</h2>\n<p>EDUCATION, EXPERIENCE, TRAINING</p>\n<p> </p>\n<p>1.     Current and valid state license as a Registered Nurse.</p>\n<p>2.     Current BLS certificate upon hire and maintain current.</p>\n<p>3.     Minimum one year experience in an applicable setting, preferred.</p>\n<p>4. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $32.00 to 44.68/hour.
