font-family: verdana, sans-serif; font-size: 14px;\">The Physical Therapist is responsible for providing rehabilitation treatment and care in the home to adult and/or geriatric patients of Suburban Home Health.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Evaluates patients and develops a rehabilitation plan within the parameters prescribed by the referring physician to improve or restore physical functioning, alleviate pain and prevent physical dysfunction.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Documents patient progress and communicates with other clinicians such as occupational therapy, speech therapy and skilled nursing on the status of patient&rsquo;s treatment plans. &nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Administers active and passive exercises to help relieve pain, increase patient strength or decrease or prevent deformity.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Directs physical therapist assistants and home health aides on the course of treatment for designated patients&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Educates and instructs patients, families and caregivers regarding treatment plans and procedures. &nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Plans discharge when patient goals have been attained and communicates proper follow-up care to patient and caregivers.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Adheres to established organizational and departmental policies, standard operating procedures, quality assessment, and safety guidelines.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Displays and maintains behavior which promotes a positive experience for patients, caregivers, referring physicians and co-workers.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\"><strong>Basic Qualifications</strong><strong>&nbsp;</strong></span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Graduate degree, or equivalent years of experience, from an accredited school of physical therapy and holder of a current permanent or temporary license from the Indiana Medical Licensing Board required.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- At least one year&rsquo;s experience in physical therapy preferred.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- CPR certification or willingness to gain CPR certification required.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Must possess attention to detail skills.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Ability to prioritize work using fundamental time management skills.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Must possess excellent interpersonal skills to foster a team environment.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Working knowledge of the healthcare system work environment and the role of the physical therapist in the &ldquo;health care team&rdquo; required.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Demonstrates knowledge and technical skill necessary for providing competent, comprehensive patient care.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0); font-family: verdana, sans-serif; font-size: 14px;\">- Must possess problem solving abilities, excellent customer service skills and Patient First characteristics.&nbsp;</span></p><p><span style=\"color: rgb(0, 0, 0);
