Description Position Summary The APRN/PA at the Center for Human Services (CHS) is responsible for delivering high-quality, evidence-based medical care to individuals with substance use disorders, with a primary focus on Opioid Treatment Program (OTP) services and Medication for Opioid Use Disorder (MOUD ) . This role supports daily medical operations through the timely completion of admissions, transfers, annual medical evaluations, and medication inductions, ensuring compliance with SAMHSA, DEA, DPH, CARF, and ASAM standards. The provider collaborates closely with nursing, counseling, and leadership teams to ensure safe, effective, and person-centered care. Schedule: Monday - Friday | 6:30 AM -12:30 PM (30 hours/week) Key Responsibilities: Clinical Duties Conduct comprehensive medical assessments for: New admissions Transfers between programs Annual physical examinations New employees Evaluate and diagnose substance use and co-occurring medical conditions Initiate and manage MOUD, including: Methadone inductions and titration Buprenorphine (Suboxone/Sublocade) initiation and maintenance Coordination for extended-release formulations when appropriate Assess appropriateness for admission based on ASAM criteria and medical necessity Medication Management Prescribe and manage medications in accordance with: Federal OTP regulations (42 CFR Part 8) State of Connecticut regulations Agency policies and protocols Provide patient education and informed consent for all medications Collaboration & Communication Work collaboratively with: Nursing staff Counselors and case managers Recovery coaches Program leadership Participate in multidisciplinary team meetings as needed Provide clinical guidance and consultation to staff Documentation & Compliance Complete all documentation in a timely, accurate, and compliant manner within the EHR Ensure documentation supports: Medical necessity Regulatory compliance (CARF, DPH, SAMHSA) Billing requirements Maintain compliance with: Confidentiality laws (HIPAA, 42 CFR Part 2) Infection control and safety standards All duties as deemed necessary by supervisor Qualifications Licensed as a Physician Assistant (PA) or Advanced Practice Registered Nurse (APRN ) in the State of Connecticut Current DEA registration with Schedule II–V authority Experience or training in addiction medicine and/or MOUD Knowledge of: ASAM criteria Medication-assisted treatment (MAT/MOUD) Harm reduction principles Core Competencies Strong clinical assessment and decision-making skills Ability to work in a fast-paced, high-volume environment Excellent communication and interdisciplinary collaboration Commitment to person-centered, trauma-informed care Cultural humility and respect for diverse populations Physical & Work Environment Clinical outpatient setting Frequent standing, walking, and interaction with clients Exposure to individuals experiencing withdrawal or acute symptoms
