In this role, you will provide state-funded case management services using a Trauma Informed Care approach, supporting youth with severe emotional disturbances while promoting stability and community-based care. We even offer pet insurance for your four-legged family members.  </li>\n</ul>\n<ul>\n <li>Early access to earned wages with Daily Pay, tuition reimbursement to help further your career and adoption assistance to help you grow your family.   </li>\n</ul>\n<p>With a collective goal to champion a culture of belonging where everyone feels valued and respected, we honor the ways people are unique and embrace what brings us together.   </p>\n<p>And, we believe equipping you with support and development opportunities is a vital part of delivering an exceptional employment experience. </p>\n<p>Find a fulfilling career and make a difference with UnityPoint Health.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Provides case management services, including assessment, treatment planning, and ongoing support for clients with behavioral health needs</li>\n <li>Conducts diagnostic assessments and develops individualized treatment plans</li>\n <li>Coordinates care and collaborates with multidisciplinary teams, including providers, social workers, and nursing staff</li>\n <li>Delivers counseling, crisis intervention, and community-based support to clients and families</li>\n <li>Monitors client progress and adjusts treatment plans based on clinical findings</li>\n <li>Maintains accurate, timely clinical documentation in compliance with regulatory standards</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education:</strong></p>\n<ul>\n <li>Requires a high school diploma or GED and a minimum of five years documented clinical experience in mental health or human services; OR</li>\n <li>An individual possessing a certificate of psychiatric rehabilitation from a DHS-approved program, plus a high school diploma or GED, plus two years' documented experience in provided mental health services; OR</li>\n <li>An individual possessing a bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or a related human service field.</li>\n</ul>\n<p><strong>License(s)/Certification(s): </strong></p>\n<ul>\n <li>Valid driver’s license.</li>\n <li>(CPI) training within 6 months of hire.</li>\n <li>Obtain certification within 2 months of hire and re-certify annually according to the requirements set forth in the Illinois Department of Healthcare and Family Services related to the  Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS)</li>\n</ul>\n<p>*Specific offers are determined by various factors, such as experience, skills, internal equity, and other business needs. The salary range listed does not include other forms of compensation which may include bonuses/incentive, differential pay, or other forms of compensation or benefits that may be applicable to this role.  </p>",
