    "description": "<h2>Overview</h2>\n<p>We are seeking a <strong>Physical Therapist</strong> to provide skilled physical therapy services to patients across <strong>Senior Living Communities</strong>. In this role, you will be instrumental in helping seniors recover, enhance mobility, and maintain their independence through tailored physical therapy treatment plans.</p>\n<p> </p>\n<p>At <strong>Covenant Home Health Agency (CHH)</strong>, our dedicated team of professionals collaborate to promote independence, enhance quality of life, and support our residents in reaching their fullest potential.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Evaluate physician’s referral and patient’s medical record.</li>\n <li>Assessing and evaluating therapeutic/rehabilitative/functional status using approved evaluation techniques, tests and observations, and participation in the development of the total plan of care.</li>\n <li>Evaluates the home environment.</li>\n <li>Determines priority needs for physical therapy.</li>\n <li>Directs physical therapy treatment.</li>\n <li>Reports to physician, patient’s response to treatment and changes in condition.</li>\n <li>Identifies patient and family/caregiver needs for other home health services and refers as necessary.</li>\n <li>May train patients in the use of prosthetic device.</li>\n <li><p>Administer a prescribed physical therapy program to restore and maintain maximum</p><p>function and prevent disability using a range of therapeutic modalities.</p></li>\n <li><p>Maintain appropriate documentation in the patient chart, and re‐evaluate patient</p><p>progress and revise treatment programs when necessary.</p></li>\n <li>Communicates physical therapy philosophy and treatment approaches and goals to patients, families and other members of the rehabilitation team.</li>\n <li>Supervise Physical Therapy Assistants according to Agency policy and state regulations.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Valid <strong>Physical Therapist license in Arizona</strong>, in good standing.</li>\n <li>Two years of Home Health experience as a Physical Therapist preferred. </li>\n <li>Possesses and maintains current CPR certification.</li>\n <li>Licensed driver with reliable vehicle and current vehicle insurance.</li>\n <li>Experience in geriatric physical therapy or working with seniors is preferred.</li>\n <li>Ability to work independently and manage time efficiently.</li>\n <li>Excellent observation, verbal and written communication skills, problem solving skills, and organizational skills.</li>\n <li>Excellent communication and interpersonal skills to work effectively with residents, families, and the healthcare team.</li>\n</ul>\n<p><strong>Schedule and service areas:</strong> </p>\n<ul>\n <li><strong>PRN</strong> to cover the West Valley & East Valley</li>\n</ul>\n<p><strong>Benefits:</strong> (Certain benefits may be exclusive to full-time and part-time positions only):</p>\n<ul>\n <li>Daily pay option available.</li>\n <li>Medical, dental, and vision insurance options.</li>\n <li>Paid time off and paid sick leave.</li>\n <li>403b company matched retirement plan.</li>\n <li>Career Growth Opportunities.</li>\n <li>The company provides a $15,000 life insurance payout, with the option to purchase additional coverage.</li>\n <li>Employee assistance program and much more!</li>\n</ul>\n<p> </p>\n<p>Covenant Home Health is a not-for-profit Christian Care Community providing years of affordable senior housing to our residents while promoting their quality of life and independence.</p>\n<p> </p>\n<p>We are a company that cares about its hard-working employees.
