The LPN is responsible for working to full scope of practice, performing work as required and functioning as an effective member of the interdisciplinary team.&nbsp;</p><p><br></p><p data-pasted=\"true\"><strong>Essential Job Functions</strong></p><p><strong>Participates:</strong></p><ul><li>As part of the PACT team, the LPN provides ongoing service coordination and links service recipients with resources.</li></ul><p><strong>Responsibilities:</strong></p><ul><li>Provide evidence-based nursing care within full scope of LPN practice</li><li>Obtain and update medical histories and medical information on the EMR, using appropriate evidence based assessment techniques and instruments during a person served visit, with the ability to pass this information onto the PACT Registered Nurse</li><li>Schedule and coordinate referrals for tests, follow up to other disciplines and utilizing community resources as needed for persons served by the team(s)</li><li>Able to assist with medication administration as assigned</li><li>Obtain and document weight, height, girth, and vital signs</li><li>Assist with implementation of new protocols and act as resource to other staff</li><li>Maintain therapeutic nurse-person served relationships</li><li>Provide updates on persons served medical status and needs to mental health treatment team and coordinate care with team</li><li>Involved in the development, management, and evaluation of the individual, with the family, and other healthcare providers in planning mutually expected outcomes</li><li>Provide health teaching, addressing topics such as healthy lifestyle, risk reduction behaviors and preventative self-care</li><li>Participate in quality improvement activities in collaboration with the PACT Registered Nurse</li><li>Participate in ongoing educational activities related to learning needs and professional issues</li><li>Serve as a person served advocate and assist him/her to advocate for themselves</li><li>Administer vaccines and IM injections deemed necessary by provider during the visit, and may draw blood samples or collect urine samples for lab testing as necessary</li><li>Actively seek out supervision from own supervisor at least monthly, and participate in group nursing practice supervision as assigned</li></ul><p><br></p><p data-pasted=\"true\"><strong>Requirements</strong></p><p><strong>Education/Certifications:</strong></p><ul><li>High school diploma or equivalent, must have graduated from an accredited nursing training program and possess and maintain licensure, in good standing, in the State of NJ as an LPN.</li><li>Current CPR Certificate required (BLS)</li></ul><p><strong>Experience:</strong></p><ul><li>Immunization course</li><li>Medication course</li><li>IM Course</li></ul><p><strong>Driver&rsquo;s License, Vehicle:</strong></p><ul><li>Valid driver&rsquo;s license required</li><li>No more than one moving violation within the past 12 months</li><li>Vehicle required</li></ul><p><strong>&nbsp;</strong><strong>EXCELLENT BENEFITS:</strong></p><ul type=\"disc\"><li>Benefits: Medical, Dental, Vision, 403b, basic life and AD&amp;D, flexible spending accounts, EAP</li><li>Eligible for medical benefits after 30 days of employment&nbsp;</li><li>Flexible work schedules, clinical training series, leadership development program</li><li>10 paid holidays (an 11<sup>th</sup> after 2 years of employment), generous vacation and sick time</li></ul><p>&nbsp;</p><p><strong id=\"isPasted\"><em>Bridgeway Behavioral Health Services was founded on a strong desire to fight disparity and injustice alongside people living with behavioral health conditions.
